摘 要:目的评价新型药学门诊服务模式对老年2型糖尿病患者的干预效果。方法选取2017年8月至2018年12月到上海市静安区中心医院药物治疗管理(MTM)药学门诊就诊的老年2型糖尿病患者42例,至少经1次MTM门诊干预后,评价患者用药依从性、生活质量、血糖管理情况。结果干预后,患者Morisky量表评分为(7.05±0.97)分,显著高于干预前的(5.56±1.89)分(P<0.05)。糖尿病特异性生活质量(DSQL)量表中,心理状态维度评分为(16.83±2.62)分,显著低于干预前的(18.95±3.32)分(P<0.05);治疗维度评分为(5.40±0.89)分,显著低于干预前的(6.24±1.14)分(P<0.05);患者的空腹血糖(FBG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1C)水平经干预后均显著降低(P<0.05)。结论基于MTM模式的创新药学门诊服务有助于提高2型糖尿病患者的用药依从性,改善其生活质量并促进其血糖水平的有效控制。Objective To evaluate the intervention effect of the new outpatient pharmacy service on the elderly patients with type 2 diabetes.Methods A total of 42 elderly type 2 diabetic patients who were treated at the MTM(medication therapy management)outpatient pharmacy of Shanghai Jing’an District Central Hospital from August 2017 to December 2018 were selected.After at least one MTM outpatient intervention,the patient’s medication compliance,quality of life and blood glucose management were evaluated.Results After intervention,the patients’Morskiy Scale score was(7.05±0.97)points,which was significantly higher than(5.56±1.89)points before the intervention(P<0.05);in the diabetes-specific quality of life(DSQL)scale,the mental state dimension score was(16.83±2.62)points,which was significantly lower than the(18.95±3.32)points before intervention(P<0.05);the treatment dimension score was(5.40±0.89)points,which was significantly lower than(6.24±1.14)points before intervention(P<0.05);the patients’fasting blood glucose(FBG),blood glucose after 2 hours of meal(2 hPG)and glycated hemoglobin(HbA1 C)levels were significantly reduced after intervention(P<0.05).Conclusion The new outpatient pharmacy service based on the MTM model can help improve the medication compliance of patients with type 2 diabetes,improve their quality of life and promote the effective control of their blood glucose level.
