摘 要:目的分析京津冀血液筛查实验室(简称血站实验室)2018年的抗-HIV ELISA检测复检率及复检符合率情况,探究各实验室抗-HIV试剂及系统的稳定性和其影响因素,为实现京津冀血站实验室同质化建设提供依据。方法通过填报调查问卷的形式,收集2018年全年京津冀地区15家血站实验室各月试剂使用情况,抗-HIV检测的标本数,每种试剂需进行重复检测标本数量和ELISA重复检测为反应性标本数量,进行比较分析。结果1)2018年京津冀15家血站实验室的抗-HIV检测复检符合率有统计学差异(P<0.05);2)不同复检策略的实验室抗-HIV复检符合率存在统计学差异(P<0.05);3)不同规模实验室抗-HIV检测复检符合率无统计学差异(P>0.05);4)8种抗-HIV检测试剂的复检符合率差异有统计学意义(P<0.05);5)试剂R1,R5,R8在不同实验室检测抗-HIV复检符合率无统计学差异(P>0.05),其余试剂各实验室间差异有统计学意义(P<0.05);6)实验室B,C,E,F,G,N使用2-3种试剂检测的抗-HIV复检符合率差异有统计学意义(P<0.05),其余各实验室试剂间复检符合率无统计学差异。结论2018年京津冀地区15家血站实验室抗-HIV检测复检符合率差异有统计学意义,除使用试剂本身的差异外,实验室内的其他因素对其也有较大影响,各实验室在抗-HIV检测中存在的问题及改进方法有待进一步研究和探讨。Objective To investigate the concordance rate of anti-HIV reactivity in initial and repeat detection in blood station laboratories from Beijing-Tianjin-Hebei region in 2018,and explore the differences in anti-HIV detection ability and influencing factors in each laboratory,so as to lay the foundation for the homogenization of the blood screening laboratories(labs)in the Beijing-Tianjin-Hebei region.Methods The monthly data,including the total number of anti-HIV detection,initial reactive(IR)and repeat reactive(RR)anti-HIV detection in each reagent of each lab were collected via questionnaires,and the differences among labs were compared and analyzed.Results 1)the concordance rates of anti-HIV reactivity between IR and RR were significantly different from each other in 2018(P<0.05);2)the concordance rates of anti-HIV reactivity between IR and RR varied among labs adopting different algorithms about repeat detection(P<0.05);3)The concordance rates of IR/RR anti-HIV in labs with different sizes has no statistical difference(P>0.05),4)The concordance rates of IR/RR anti-HIV among 8 kinds of anti-HIV reagents were statistically different(P<0.05);5)as to the same reagents adopted by different labs,the IR/RR concordance rates of reagents R1/R5/R8 showed little difference(P>0.05),while the other reagents showed statistical difference(P<0.05);6)The IR/RR concordance rates of anti-HIV in labs B,C,E,F,G,N,using 2—3 kinds of reagents,showed statistical differences(P<0.05),while the concordance rates of other labs showed little difference(P>0.05).Conclusions The IR/RR concordance rates of anti-HIV of 15 blood screening laboratories from Beijing-Tianjin-Hebei region in 2018 were statistically different from each other.In addition to the difference inherent in reagents themselves,other factors in the labs also had a great impact on the concordance rates of anti-HIV.The existing problems and strategies for further improvement in anti-HIV detection of each laboratory need to be further studied and discussed.
