健脾补肾通络法联合强的松治疗进展型IgA肾病的效果

摘　　要：目的研究健脾补肾通络法联合强的松治疗进展型IgA肾病(IgAN)的效果。方法选取2002年1月~2017年3月就诊于上海中医药大学附属龙华医院门诊或病房住院的IgAN患者69例,其中37例单用健脾补肾通络法治疗为中医组,32例联合强的松治疗为中西医组。比较两组治疗前后空腹血肌酐、血尿素氮、血清白蛋白、肾小球滤过率、血尿酸及24 h尿蛋白等指标,并观察两组疗效。结果治疗后,两组血肌酐、24 h尿蛋白均较治疗前降低,血清白蛋白、肾小球滤过率均较治疗前升高,差异有统计学意义(P <0.05)。治疗后,中西医组肾小球滤过率高于中医组,24 h尿蛋白低于中医组,差异有统计学意义(P <0.05)。中西医组总有效率高于中医组,差异有统计学意义(P <0.05)。结论健脾补肾通络法联合强的松能进一步稳定IgAN患者的肾脏功能,降低蛋白尿。Objective To study effect of invigorating the spleen and kidney and dredging channels method combined with Prednisone in the treatment of progressive IgA nephropathy(IgAN).Methods From January 2002 to March 2017,69 patients with IgAN were selected from the Outpatient Department or Ward of Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine.Among them,37 patients treated with the method of invigorating the spleen and kidney and dredging channels were taken as Chinese medicine group,32 patients treated with invigorating the spleen and kidney and dredging channels method combined with Prednisone were taken as Chinese and Western medicine group.The indexes of fasting serum creatinine,blood urea nitrogen,serum albumin,glomerular filtration rate,serum uric acid and 24 h urine protein were compared between two groups before and after treatment,and the curative effect of two groups was observed.Results After treatment,the serum creatinine and 24-h urinary protein of two groups were lower than before treatment,the serum albumin and glomerular filtration rate were higher than before treatment,the differences were statistically significant(P<0.05).After treatment,the glomerular filtration rate of Chinese and Western medicine group was higher than that of Chinese medicine group,and the 24-h urine protein of Chinese and Western medicine group was lower than that of Chinese medicine group,the differences were statistically significant(P<0.05).Total effective rate of Chinese and Western medicine group was higher than that of Chinese medicine group,the difference was statistically significant(P<0.05).Conclusion Invigorating the spleen and kidney and dredging channels method combined with Prednisone can further stabilize the renal function of IgAN patients and reduce proteinuria.
