基 金:多排螺旋CT后处理技术的联合运用在规避超小范围外伤性主动脉夹层(AD)中的应用价值(编号:WJ2017F100)。
摘 要:目的:分析多层螺旋计算机断层扫描技术(CT)联合超声及后处理技术在外伤性主动脉夹层诊断中的应用价值。方法:回顾性研究2015年12月至2018年7月期间湖北省钟祥市人民医院85例外伤性主动脉夹层患者影像学资料和临床资料。对比多层螺旋CT各种后处理技术(CPR、MPR、VR、MIP)在真假腔、内膜瓣及破口上的显示率;对比超声心动图、多层螺旋CT单独使用与二者联合诊断主动脉夹层(AD)准确率;以手术结果为准,对比单独使用超声心动图、多层螺旋CT和联合超声心动图与多层螺旋CT在AD各类型上的诊断准确率。结果:在真假腔、内膜瓣显示率上,增强扫描、CPR及MPR均高达100%。MPR和CPR在内膜瓣显示率、真假腔显示率、破口显示率上均较VR(97.65%、97.65%、16.48%)和MIP(80.00%、80.00%、15.29%)高,P<0.05,R在内膜瓣显示率和真假腔显示率上较MIP高,P<0.05。多层螺旋CT诊断AD准确率(98.82%)较超声心动图(76.47%)高,联合检查诊断AD准确率(100.00%)较多层螺旋CT高,P<0.05。以手术结果为准,联合检查在AD DeBakeyⅠ、Ⅰ、Ⅲ上诊断正确率均为100%,联合检查在DeBakeyⅡ型上诊断正确率(100.00%)较超声心动图(91.67%)和多层螺旋CT(66.67%)高,在DeBakeyⅢ型上诊断正确率较超声心动图(51.72%)高,P<0.05。结论:多层螺旋CT联合超声心动图诊断外伤性主动脉夹层准确率高。Objective:To analyze the value of multi-slice spiral computed tomography(CT)combined with ultrasound and postprocessing techniques in the diagnosis of traumatic aortic dissection.Methods:A retrospective study was conducted.During the period from December 2015 to July 2018,the authors included 85 patients with traumatic aortic dissection in their hospitals,and analyzed their imaging data and clinical data.Both were multi-slice spiral CT and echocardiography.Contrast the multi-slice spiral CT various post-processing techniques(CPR,MPR,VR,MIP)on the true false lumen,intimal flap and rupture;contrast echocardiography,multi-slice spiral CT alone and the combination of the two The accuracy of AD was based on the surgical results,compared with the diagnostic accuracy of echocardiography,multi-slice spiral CT and combined echocardiography and multi-slice spiral CT on various types of AD.Results:In the true and false lumen,intimal flap display rate,enhanced scanning,CPRand MPR are up to 100%.MPR and CPR were higher in VR(97.65%,97.65%,16.48%)and MIP(80.00%,80.00%,15.29%),P<0.05,R was higher than MIP in the rate of intimal flap display and true false lumen display,P<0.05.The accuracy of multi-slice spiral CT diagnosis of AD(98.82%)was higher than that of echocardiography(76.47%).The accuracy of combined diagnosis of AD(100.00%)was higher than that of spiral CT,P<0.05.According to the surgical results,the diagnostic accuracy of the combined examination on AD DeBakey I,I,III is 100%,the diagnostic accuracy of the combined examination on DeBakey type II(100.00%)is higher than echocardiography(91.67%)and multi-slice spiral CT(66.67%)high,the diagnostic accuracy rate on DeBakey type III was higher than that of echocardiography(51.72%),P<0.05.Conclusion:Multi-slice spiral CT combined with echocardiography has a high accuracy in the diagnosis of traumatic aortic dissection,which can be used as the preferred post-processing technique.
