摘 要:【目的】探讨惠州地区青少年碘营养状况及其体质量指数(BMI)与甲状腺相关抗体的相关性.【方法】分别于2016年12月和2017年12月随机抽取了惠州市4个中学共1207名青少年,检测其尿碘、身高、体质量、促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)和促甲状腺激素受体抗体(TRAb).【结果】惠州地区青少年的尿碘中位数(MUI)为205.93μg/L,男、女尿碘水平比较差异无统计学意义(P>0.05,Z=-0.97);尿碘水平与TPOAb呈正相关(R=0.42,P<0.01),血清TSH水平与BMI呈正相关(R=0.24,P<0.01);青少年超重人群与正常BMI人群尿碘水平差异无统计学意义(P>0.05,Z=-0.10),一级亲属有甲状腺功能亢进或者甲状腺功能减退病史的青少年人群尿碘水平明显升高(P<0.001,Z=-4.91).【结论】青少年尿碘水平可能与甲状腺自身免疫性疾病有关,但与青少年肥胖无关,尿碘浓度监测对有明显甲状腺疾病聚集家庭的青少年具有重要意义.【Objective】To study the correlation between iodine nutritional status and thyroid related antibody in adolescents in Huizhou.【Method】 A total of 1207 adolescents from 4 high schools in Huizhou were selected randomly in December 2016 and December 2017 respectively,and their urinary iodine concentration,height,weight,serum TSH,FT3,FT4,thyroid peroxidase antibodies(TPOAb),thyroglobulin antibodies(TgAb),and thyrotrophin receptor antibody(TRAb)were detected.【Results】The median urinary iodine(MUI)of adolescents was 205.93μg/L in Huizhou region.There was no significant difference in urinary iodine level between boys and girls(P>0.05,Z=-0.97).Urinary iodine level was positively correlated with TPOAb(R=0.42,P V0.001).Serum TSH level was positively correlated with BMI(R=0.24,P V0.01).There was no significant difference in urinary iodine level between overweight adolescents and normal BMI population(P>0.05.Z=-0.10).Urinary iodine level was significantly higher in first-degree relatives who have history of hyperthyroidism or hypothyroidism of adolescents(P<0.001,Z=-4.91).【Conclusion】Urinary iodine levels in adolescents may be associated with thyroid autoimmune diseases,however,it does not affect obesity in adolescents.Urinary iodine concentration monitoring is important to adolescents with thyroid disease clustering families.
