血栓弹力图在内科疾病成分输血中的应用研究

摘　　要：目的研究血栓弹力图(TEG)检测在内科重症患者进行成分输血中的应用价值。方法选取临床需输血浆和/或血小板的非血液病、非手术重症患者60例,采集患者血样分别进行TEG检测和凝血项检测,将TEG检测结果和凝血项指标进行相关分析。结果活化部分凝血活酶时间、血浆凝血酶原时间与凝血反应时间呈正相关(r=0.44、0.34,P<0.05),纤维蛋白原与最大振幅呈正相关(r=0.34,P<0.05),血小板与a角、最大振幅呈正相关(r=0.46、0.68,P<0.05)。结论血栓弹力图检测与凝血项检测指标呈明显正相关,对内科重症患者合理进行成分输血具有指导意义。Objective To study the detection of hromboelastography(TEG)select the required plasma transfusion and clinical application value.Methods Plasma and platelet transfusion of blood components in critically ill patients in the department of internal medicine blood dease,non-operation 60 critically ill patients,collecting blood samples of patients were detected by TEG and the coagulation of blood test,the TEG test results and coagulation test the correlation analysis.Results APTT,PT and coagulation reaction time(R)was positive correlation(r=0.44,0.34,P<0.05),FIB and MA was positive correlation(r=0.34,P<0.05),PLT and a angle,MA was positive correlation(r=0.46,0.68,P<0.05).Conclusion Thrombelastography show significant positive correlation detection and coagulation test index of severe patients of Department of internal medicine,reasonable component blood transfusion has guiding significance.
