责任制助产护理结合导乐陪伴分娩模式在初产妇自然分娩中的应用

摘　　要：目的观察责任制助产护理结合导乐陪伴分娩模式在初产妇自然分娩中的临床应用效果。方法选取2018年1-6月在该院实施责任制助产护理结合导乐陪伴的分娩服务自然分娩的220例自然分娩初产妇作为试验组;另选取同期在该院实施全程责任制助产护理自然分娩的235例初产妇作为对照组。比较两组初产妇的分娩方式、分娩结局、分娩疼痛、满意度等的差异。结果试验组自然分娩率、产妇满意度明显高于对照组,而阴道助产率、中转剖宫产率、产时及产后2 h出血均低于对照组,差异均有统计学意义(χ^2=7.904、4.191、5.099、3.285、4.749,P<0.05);试验组产妇会阴侧切率、会阴Ⅰ度、会阴Ⅱ度、新生儿窒息率、第1产程疼痛感、第2产程病痛感均明显低于对照组,差异均有统计学意义(χ^2=14.992、4.193、2.077、3.787、5.378、8.876,P<0.05),而两组会阴完整差异无统计学意义(P>0.05)。结论在初产妇自然分娩过程中,实施责任制助产护理结合导乐陪伴分娩模式不仅可有效降低会阴侧切率和中转剖宫产率,有利于改善母婴分娩结局,还能减轻产妇的分娩疼痛感,提高产妇满意度。Objective To observe the clinical effect of responsibility midwifery nursing combined with doula accompanying delivery mode in natural delivery of primiparas.Methods A total of 220 primipara who gave birth naturally in the hospital from January to June 2018 were selected as the study group;in addition,a total of 235 primipara in the same period were selected as the control group,and the whole-process responsibility midwifery nursing mode was adopted at this stage.The differences of delivery mode,delivery outcome,delivery pain and satisfaction between the two groups were compared.Results The natural delivery rate and maternal satisfaction in the experimental group were significantly higher than those in the control group,the rates of vaginal midwifery,cesarean section,and bleeding at birth and 2 h postpartum were all lower than those of the control group,and the differences were statistically significant(χ^2=7.904,4.191,5.099,3.285,4.749,P<0.05);the lateral perineal incision rate,perinealⅠdegree,Ⅱdegree,neonatal asphyxia rate,and the course of labor in the experimental group was significantly lower than that of the control group(χ^2=14.992,4.193,2.077,3.787,5.378,8.876,P<0.05),but there was no significant difference in perineal integrity between the two groups(P>0.05).Conclusion In the natural delivery process of primipara,the implementation of responsibility midwifery nursing combined with doula delivery mode can not only effectively reduce the rate of lateral perineal section and cesarean section,but also benefit the delivery rate,it can also reduce maternal pain and improve maternal satisfaction.
