摘 要:目的探讨不同模式鼻咽部放疗对人工耳蜗设备性能的影响,以指导人工耳蜗植入者放疗方案的制定和选择。方法将三个人工耳蜗植入体按经典的开放乳突-面隐窝经圆窗入路植入尸头建立人工耳蜗植入模型(简称:模型A、B、C),并模拟鼻咽癌放射野实施两种不同的放疗方案,模型A:模拟鼻咽癌延长性调强放射,以植入的人工耳蜗的位置为中心靶区,放疗总剂量80 Gy,每次2 Gy,分40次完成,每周5次;模型B:模拟鼻咽癌常规调强放射,以鼻咽部为中心靶区,放射总剂量70 Gy,每次约2 Gy,分33次完成,每周5次;模型C:不做任何放疗处理;完成放疗后分别检测三个模型的人工耳蜗电子元件的电极阻抗及输出电流辐度。结果两种模拟正常剂量的鼻咽癌放疗方案照射人工耳蜗后,模型A、B植入体都能在正常条件下启动,电极阻抗正常;实验前模型A、B、C的输出电流幅度的平均偏差百分比分别为0.61%、0.50%、0.58%;实验后分别为0.52%、0.46%、0.54%;均在正常范围内(±10%)。结论两种不同模式鼻咽部放疗方案短期内对人工耳蜗设备性能无明显影响。Objective To investigate the effects of different nasopharyngeal radiotherapy modes on the performance of cochlear implants,in order to guide the formulation and selection of postoperative radiotherapy for cochlear implants.Methods Three cochlear implants product by Nurotron were implanted into the model(Model A, B, C) with the way of classic open mastoid-face crypt via a round window.Then two different radiotherapy were implemented.Model A was treated byprolonged intensity modulated radiotherapy(IMRT) sameas nasopharyngeal carcinoma. But the central target area was the implanted electronic cochlea.The total dose was about 80 Gy with 2 Gy each time, 40 times to complete, 5 times a week. Model B was treated by nomal IMRT in the same way of nasopharyngeal carcinoma.The central target area was nasopharynx.The total dose was about 70 Gy with 2 Gy each time, 33 times to complete, 5 times a week.There was no radiotherapy treatment for Model C.Finally the stability of the artificial cochlear electronic components were tested by instrument. Results After irradiation of cochlear implants with normal dose of nasopharyngeal carcinoma radiotherapy, the implants could start under normal conditions and the electrode impedance was not damage. The average deviation percentage of output current amplitude of ModelA, B and C before the experiment was 0.61%, 0.50% and 0.58%, respectively, and after the experiment was 0.52%. 0.46% and 0.54%, respectively. All data were in the normal range(+10%). Conclusion Two different radiotherapy schemes in nasopharynx have no significant effect on the performance of cochlear implants in a short time.
