氯吡格雷联合阿司匹林肠溶片治疗不稳定型心绞痛的临床疗效和安全性分析

摘　　要：目的研究分析氯吡格雷和阿司匹林肠溶片联合服用在治疗不稳定型心绞痛中的临床疗效和安全性。方法选择2017年1月至2018年12月在我科治疗的92例不稳定性心绞痛患者为研究对象,按照随机原则分成两组,每组46例。两组患者入院后予以持续低流量吸氧、止痛、扩张冠状动脉等对症治疗,对照组患者予以口服阿司匹林肠溶片,观察组患者同时口服氯吡格雷和阿司匹林肠溶片,观察比较两组患者的临床疗效和不良反应情况。结果对照组患者临床总有效率为78.26%,观察组患者临床总有效率为91.30%,观察组患者临床总有效率显著高于对照组。对照组患者不良反应发生率4.35%,观察组患者不良反应发生率为6.52%,两组间差异无统计学意义。治疗后两组患者疼痛发作频率均低于治疗前,疼痛持续时间均短于治疗前,但治疗后观察组患者疼痛发作频率低于对照组,疼痛持续时间也短于对照组。结论氯吡格雷联合阿司匹林肠溶片能够更好抑制血小板的聚集和黏附,预防血栓的形成,明显改善不稳定型心绞痛患者的预后,临床效果显著,不良反应发生率低,值得临床应用。Objective To study and analyze the clinical efficacy and safety of clopidogrel and aspirin enteric-coated tablets in the treatment of unstable angina pectoris.Methods Ninety-two patients with unstable angina pectoris treated in our department from January 2017 to December 2018 were selected as the study subjects and randomly divided into two groups,46 cases in each group.After admission,two groups of patients were given symptomatic treatment,such as continuous low flow oxygen inhalation,pain relief,dilatation of coronary artery.The control group was given oral aspirin enteric-coated tablets.The observation group was given clopidogrel and aspirin enteric-coated tablets at the same time.The clinical efficacy and adverse reactions of the two groups were observed and compared.Results The total clinical effective rate of the control group was 91.30%,and that of the observation group was 78.26%.The total clinical effective rate of the observation group was significantly higher than that of the control group.The incidence of adverse reactions was 4.35%in the control group and 6.52%in the observation group.There was no significant difference between the two groups.After treatment,the frequency of pain onset in both groups was lower than that before treatment,and the duration of pain was shorter than that before treatment.However,after treatment,the frequency of pain onset in observation group was lower than that in control group,and the duration of pain was shorter than that in control group.Conclusion Clopidogrel combined with aspirin enteric-coated tablets can better inhibit platelet aggregation and adhesion,prevent thrombosis,and significantly improve the prognosis of patients with unstable angina pectoris.The clinical effect is remarkable,and the incidence of adverse reactions is low.It is worthy of clinical application.
