摘　　要：目的:探讨硬膜外阻滞复合全凭麻醉对老年右半结肠癌患者术后拔管时间及免疫功能的影响。方法:选取2017年1月至2019年6月我院老年右半结肠癌患者200例,按麻醉方案不同分复合组(n=100)、参照组(n=100)。均行腹腔镜全肠系膜切除术治疗,参照组采用全凭麻醉,复合组在参照组基础上采用硬膜外阻滞麻醉。比较两组术后苏醒时间、拔管时间、术前、术后6h、24h免疫功能[总T淋巴细胞(CD3+)、免疫状态(CD4^+/CD8^+)]。结果:复合组术后苏醒时间、拔管时间均短于参照组(P<0.05);术后6h、24h复合组CD3+、CD4^+/CD8^+均高于参照组(P<0.05)。结论:硬膜外阻滞复合全凭麻醉可显著降低对老年右半结肠癌患者机体免疫功能的影响,缩短术后苏醒及拔管时间,促进患者机体恢复。Objective:To investigate the effect of epidural block combined with total anesthesia on the extubation time and immune function of elderly patients with right colon cancer.Methods:From January 2017 to June 2019,200 elderly patients with right colon cancer in our hospital were divided into compound group(n=100)and reference group(n=100)according to different anesthesia schemes.All patients were treated by laparoscopic total mesentery resection.The control group was treated by total anesthesia,and the compound group was treated by epidural anesthesia on the basis of the control group.The recovery time,extubation time,preoperative and postoperative immune function(CD3+,CD4^+/CD8^+)were compared between the two groups.Results:The recovery time and extubation time of the compound group were shorter than those of the reference group(P<0.05).CD3+,CD4^+/CD8^+in the compound group were higher than those in the reference group(P<0.05).Conclusion:Epidural block combined with total anesthesia can significantly reduce the effect on the immune function of elderly patients with right colon cancer,shorten the time of recovery and extubation,and promote the recovery of patients.
