摘  要:目的:通过观察针刺捻转补泻手法对自发性高血压大鼠(SHR)海马形态的改变、糖皮质激素受体(GR)表达及下丘脑-垂体-肾上腺皮质(HPA)轴各组分表达的影响,探讨其调控血压的中枢-外周机制。方法:48只雄性SHR大鼠随机等分为模型组、捻转补法组与捻转泻法组,16只WKY大鼠作为空白对照组。各针刺组分别采用不同的针刺手法干预双侧太冲穴,模型组与空白组不做针刺处理,各组于针刺前及针刺后定期测量血压。实验第16天采用HE染色法观察海马组织病理性改变,采用Elisa法检测海马GR及HPA轴各组分的表达。结果:与模型组比较,捻转泻法组和捻转补法组均能明显降低SHR大鼠血压(P<0.01或P<0.05),各针刺组海马GR含量明显降低(P<0.05),HPA轴各组分表达量明显降低(P<0.05);且捻转泻法组较捻转补法组降压效果和良性调控HPA轴作用更明显(P<0.05)。结论:针刺捻转补泻手法可以通过调节海马中GR含量表达、抑制HPA轴活动,起到降压作用,这可能是针刺捻转补泻手法降压的机制之一。Objective:To observe the effects of twirling reinforcing-reducing manipulation(TRRM)on the changes in hippocampal morphology and the expression of glucocorticoid receptor(GR)and hypothalamic-pituitary-adrenal cortex(HPA)axis in spontaneously hypertensive rats(SHR),and to explore its central-peripheral mechanism of regulating blood pressure.Methods:48 male SHR rats were randomly divided into the model group,the twirling reinforcing group and the twirling reducing group,another 16 WKY rats were taken as the blank control.The acupuncture groups were intervened with corresponding manipulation of reinforcing and reducing on Taichong(LR3).Blood pressure was measured before and after acupuncture.On the 16th day of the experiment,the pathological changes of hippocampus were observed by HE staining,and the expression of GR in hippocampus and HPA axis components were detected by ELISA method.Results:Blood pressure showed a significant decrease both in the twirling reinforcing group and the twirling reducing group compared to that in the model group(P<0.01,P<0.05).The expression of GR in hippocampus and HPA axis components were significantly decreased in each acupuncture group(P<0.05).The twirling reducing group showed better effects on lowering blood pressure and benignantly regulating the HPA axis than the twirling reinforcing group did(P<0.05).Conclusion:The TTRM can regulate the expression of GR in the hippocampus to inhibit the activity of HPA axis,and play a role in lowering blood pressure,which may be one of the mechanisms of TTRM on anti-hypertensive effects.
