摘 要:目的了解伤口分泌物分离的社区获得性耐甲氧西林金黄色葡萄球菌(CA-MRSA)与医院获得性耐甲氧西林金黄色葡萄球菌(HA-MRSA)的耐药特点,探讨其耐药基因及耐消毒剂基因的携带情况。方法收集某院2016年1月-2017年12月伤口分泌物分离的耐甲氧西林金黄色葡萄球菌(MRSA)76株,采用聚合酶链反应(PCR)方法检测其耐药基因和耐消毒剂基因。结果共分离到MRSA 76株,其中CA-MRSA 52株,HA-MRSA 24株。52株CA-MRSA对四环素、庆大霉素、红霉素的耐药率分别为36.54%、1.92%、86.54%,24株HA-MRSA对四环素、庆大霉素、红霉素的耐药率分别为50.00%、12.50%、75.00%,HA-MRSA和CA-MRSA中,耐药基因tetM、ermA及耐消毒剂基因(qacA/B)的携带率均较高。结论伤口分泌物分离的CA-MRSA与HA-MRSA菌株耐药严重,均可携带多个耐药基因和耐消毒剂基因,应引起重视。Objective To understand the drug resistance characteristics of community-acquired methicillin-resistant Staphylococcus aureus(CA-MRSA)and hospital-acquired methicillin-resistant Staphylococcus aureus(HA-MRSA)isolated from wound secretions in this region,and to explore the carrying of drug resistance genes and disinfectant resistance genes.Methods 76 strains of methicillin-resistant Staphylococcus aureus(MRSA)isolated from the wound secretion of a hospital from January 2016 to December 2017 were collected,and their drug resistance genes and disinfectant resistance gene were detected by polymerase chain reaction(PCR).Results A total of 76 strains of MRSA were isolated in this study,including 52 strains of CA-MRSA and 24 strains of HA-MRSA.The resistance rates of 52 strains of CA-MRSA to tetracycline,gentamicin and erythromycin were 36.54%,1.92%and 86.54%.The resistance rates of 24 strains of HA-MRSA to tetracycline,gentamicin and erythromycin were 50.00%,12.50%and 75.00%.The detection rates of tetM,ermA and carrying rate of the disinfectant resistance gene(qacA/B)of 76 strains of MRSA were high.Conclusion The CA-MRSA and HA-MRSA strains isolated from wound secretions have serious drug resistance carry multiple drug resistance genes and disinfectant resistance genes,which should be paid attention to by the health department.
