摘　　要：目的探讨痤疮患者血清中多态上皮黏液素(PEM)和TLRs信号表达活化与患者病情的关系。方法将2017年1月-2018年12月本院收治的168例痤疮患者作为研究对象,设为观察组,其中轻度、中度、重度痤疮分别为52例、50例、66例。并选择同期本院收治的60例健康体检者作为对照组。采用定量PCR检测技术对2组患者、不同病情痤疮患者PEM、TLR2、TLR9表达水平的差异,并分析影响痤疮患者病情的相关因素。结果观察组PEM、TLR2、TLR9信号mNRA表达显著高于对照组(P<0.05);重度痤疮患者PEM、TLR2、TLR9信号mNRA表达显著高于中度、轻度痤疮患者(P<0.05);中度痤疮患者PEM、TLR2、TLR9信号mNRA表达显著高于轻度痤疮患者(P<0.05);PEM、TLR2、TLR9信号表达水平与性别、年龄、糖尿病、病程无明显相关(P>0.05),与糖皮质激素使用史、IFN-γ、IL-6、IL-4、TNF-α呈正相关(P<0.05)。结论痤疮患者常伴有PEM、TLR2、TLR9信号高表达,病情越重,信号表达越高,可用于指导痤疮病情严重度的评估和治疗,值得临床研究。Objective To investigate the relationship between activation of polymorphic epithelial mucin,TLRs and conditions in patients with acne.Methods A total of 168 patients with acne admitted to our hospital from January 2017 to December 2018 were enrolled in the observation group,including 52 mild patients,50 moderate patients,and 66 severe patients.Another 60 healthy people who were admitted to our hospital in the same period were selected as the control group.The quantitative expression of PEM,TLR2 and TLR9 in patients with different conditions and acne patients were analyzed by quantitative PCR.The factors affecting the condition of patients with acne were analyzed.Results The expression of mNRA in PEM,TLR2 and TLR9 in the observation group was significantly higher than that in the control group(P<0.05).The expression of mNRA in PEM,TLR2 and TLR9 in patients with severe acne was significantly higher than that in patients with moderate and mild acne(P<0.05).The expression of mNRA in PEM,TLR2 and TLR9 in patients with moderate acne was significantly higher than that in patients with mild acne(P<0.05).The levels of PEM,TLR2 and TLR9 were not significantly correlated with gender,age,diabetes and disease duration(P>0.05),and positively correlated with glucocorticoid use history,IFN-γ,IL-6,IL-4 and TNF-α(P<0.05).Conclusion Patients with acne often have high expression of PEM,TLR2,and TLR9 signals.The signal expression increases with severity,showing that they can be used to guide the severity assessment and treatment of acne.It is worthy of clinical research.
