摘　　要：目的观察麝香乌龙丸对膝关节骨关节炎(knee osteoarthritis,KOA)患者血清中白细胞介素1(interleukin-1,IL-1)、转化生长因子β1(transforming growth factor-β,TGF-β1)、基质金属蛋白酶3(matrix metalloproteinase3,MMP3)、基质金属蛋白酶抑制剂1(tissue inhibitors of metalloproteinase1,TIMP-1)表达的影响。方法采用前瞻性队列研究方法,观察对象来自于2016年3月至2018年6月华北理工大学附属医院风湿免疫科门诊的KOA患者,应用随机数字表分为两组,即治疗组(麝香乌龙丸治疗)46例和对照组(抗骨增生片治疗)46例。治疗组采用麝香乌龙丸治疗,每次30粒,2次/d;对照组采用抗骨增生片治疗,每次4片,2次/d。治疗周期均为8周。取治疗组和对照组患者血清,采用酶联免疫吸附试验法检测血清中IL-1、TGF-β1、MMP3、TIMP-1的含量,并统计分析两组治疗后相关指标的差异。结果治疗组完成46例,对照组完成44例。治疗后,IL-1水平分别为治疗组(30.99±2.80)μg/L、对照组(32.90±2.84)μg/L,两组间比较差异有统计学意义(t=3.21,P<0.001);TGF-β1水平分别为治疗组(214.87±17.30)μg/L、对照组(208.85±15.53)μg/L,两组间比较差异无统计学意义(t=1.73,P=0.08);TIMP-1水平治疗组(139.60±6.79)μg/L、对照组(135.09±6.16)μg/L,两组间比较差异有统计学意义(t=3.29,P<0.001);MMP3水平治疗组(1.19±0.10)μg/L、对照组(1.23±0.06)μg/L,两组间比较差异有统计学意义(t=2.28,P=0.03);TIMP1/MMP3比值治疗组111.04±8.81、对照组118.46±12.06,差异有统计学意义(t=3.34,P<0.001)。结论麝香乌龙丸可以降低IL-1、MMP3水平;升高TGF-β1、TIMP-1和TIMP1/MMP3比值的表达水平,从而起到保护关节软骨、改善KOA患者膝关节功能的作用。Objective To observe the effects of Shexiang Wulong pill on serum interleukin-1(IL-1),transforming growth factor-β(TGF-β1),matrix metalloproteinase-3(MMP3),matrix tissue inhibitors of metalloproteinase 1(TIMP-1)in patients with knee osteoarthritis(KOA)expression.Methods Using prospective cohort study method,the subjects were KOA patients from Department of Rheumatology and Immunology clinic of North China University of Science and Technology Affiliated Hospital from March 2016 to June 2018.According to random number table The patients were divided into two groups:the treatment group(treated with shexiangwulong pill)with 46 cases and the control group(Kanggu Zengsheng tablet)with 46 cases.The treatment group was treated with Shexiang Wulong pill,30 capsules per time,twice a day;the control group was treated with Kanggu Zengsheng tablet,four tablets per time,twice a day.The treatment period was 8 weeks.The levels of IL-1,TGF-β1,MMP3 and TIMP-1 in the serum of the patients in the treatment group and the control group were measured by enzyme-linked immunosorbent assay,and the differences of relevant indexes between the two groups were statistically analyzed.Results Forty-six cases were completed in the treatment group and 44 cases were completed in the control group.After treatment with Shexiang Wulong pill and Kanggu Zengsheng tablet,the levels of IL-1 were(30.99±2.80)μg/L,(32.90±2.84)μg/L in the treatment group and the control group,resectively(t=3.21,P<0.001).There was no significant difference of TGF-β1 levels between the treatment group((214.87±17.30)μg/L)and the control group((208.85±15.53)μg/L,(t=1.73,P=0.08).The levels of TIMP-1 were(139.60±6.79)μg/L,(135.09±6.16)μg/L in the treatment group and the control group,resectively(t=3.29,P<0.001).The levels of MMP3 were(1.19±0.10)μg/L,(1.23±0.06)μg/L in the treatment group and the control group,resectively(t=2.28,P=0.03).The rate of TIMP1/MMP3 were 111.04±8.81,118.46±12.06 in the treatment group and the control group,resectively(t=3.34,P<0.001)
