摘 要:目的分析[足母]甲瓣联合第2足趾组合移植Ⅰ期塑形再造拇指的临床价值。方法选择2017年6月至2019年6月本院接收并治疗21例拇指缺损患者,采用[足母]甲瓣联合第2足趾组合移植Ⅰ期塑形再造拇指治疗方法,对比患者接受治疗前后再造拇指成活率、坏死情况以及对比治疗前后感觉运动功能,分析外观满意度。结果21例患者接受相应治疗后,成活20例,甲床部位坏死1例;且患者治疗后感觉运动功能高于治疗前,满意度较高(P<0.05)。结论针对拇指缺损患者临床采用踇甲瓣联合第2足趾组合移植Ⅰ期塑形再造拇指治疗效果显著,可有效改善缺陷情况,同时提高外观美观,确保感觉运动功能正常,对提高患者治疗效果具有显著意义。Objective To investigate the clinical value of hallux skin-nail flap and second phalanx complex transfer for first-stage plastic reconstruction of thumb.Methods Included in this study were 21 patients with thumb defects treated in our hospital between June 2017 and June 2019.The patients received hallux skin-nail flap grafting and second phalanx complex transfer for first-stage plastic reconstruction of the thumb.The transplant survival and necrosis,as well as sensori motor functions before and after treatment were compared.Cosmetic satisfaction was also recorded.Results Of 21 patients after surgery,the transplant survived well in 20,except for nail bed necrosis found in one case.The sensor imotor function after treatment was improved compared with baseline.Fairly good cosmetic satisfaction was reported(P<0.05).Conclusion For the patients with thumb defects,first-stage plastic reconstruction with hallux skin-nail flap and second phalanx complex transfer may result in significant clinical outcome.The treat-ment may offer effective defect repair,good cosmetics and normal sensorimotor function,and therefore is clinically valuable.
