3.0T多参数MRI在前列腺病变术前良恶性鉴别诊断中的应用价值分析

摘　　要：目的分析3.0T多参数磁共振成像(MRI)在前列腺病变术前良恶性鉴别诊断中的应用价值。方法选取110例前列腺病变患者,按照病理结果分为前列腺癌组(n=52)与前列腺增生组(n=58),比较两组术前3.0T MRI扫描参数[表观扩散系数(ADC)、(胆碱+肌酸)/枸橼酸盐波峰谱线(CC/C)、血管外细胞外容积分(Ve)、容量转移常数(Ktrans)与速率常数值(Kep)、初始曲线下面积(iAUC)],并观察3.0T多参数MRI诊断准确性。结果前列腺癌组ADC明显小于前列腺增生组(P<0.05),CC/C、Ve、Ktrans、Kep、iAUC显著高于前列腺增生组(P<0.05);3.0T多参数MRI鉴别前列腺癌与前列腺增生灵敏度94.23%,特异度96.55%,准确性95.45%,Kappa值0.91。结论3.0T多参数MRI能够准确鉴定前列腺病变术前良恶性,为患者治疗提供可靠指导。Objective To analyze the application value of 3.0T multi-parameter magnetic resonance imaging(MRI)in the differential diagnosis of preoperative benign and malignant prostate lesions.Methods 110 patients with prostate lesions admitted to our hospital were selected and divided into prostate cancer group(n=52)and benign prostatic hyperplasia group(n=58)according to the pathological results.The preoperative 3.0T MR scan parameters[apparent diffusion coefficient(ADC),(choline+creatine)/citrate peak line(CC/C),extravascular extracellular volume(Ve),capacity transfer constant(Ktrans)and rate constant(Kep),initial area under the curve(iAUC)]were compared between the two groups,and the diagnostic accuracy of 3.0T multi-parameter MRI was observed.Results The ADC in prostate cancer group was significantly smaller than that in benign prostatic hyperplasia group(P<0.05)while the CC/C,Ve,Ktrans,Kep and iAUC were significantly higher than those in benign prostatic hyperplasia group(P<0.05).The sensitivity,specificity,accuracy and Kappa value of 3.0T multi-parameter MRI were 94.23%,96.55%,95.45% and 0.91 in the identification of prostate cancer and benign prostatic hyperplasia.Conclusion 3.0T multi-parameter MRI can accurately identify the preoperative benign and malignant prostate lesions,and provide reliable guidance for the treatment of patients.
