摘 要:目的:应用静息态脑功能磁共振技术中局部一致性方法探究颈源性头痛发生的中枢机制以及病人脑区的改变程度。方法:采用GE 3.0T磁共振对13例颈源性头痛病人(cervicogenic headache,CEH)组和13例健康志愿者(health control group,HC)组进行脑功能磁共振扫描并进行数据处理统计。将CEH组差异脑区的局部一致性值(regional homogeneity,ReHo)与病人视觉模拟评分法(visual analogue scale,VAS)评分及病程进行相关分析。结果:CEH组与HC组相比,ReHo值增高的脑区有双侧楔前叶、左侧胼胝体压部、左侧背侧丘脑、左侧海马旁回、右侧岛叶、右侧枕中回以及双侧小脑半球;ReHo值降低的脑区有双侧额上回-额中回-前扣带回、右侧中央后回、右侧楔前叶-后扣带回、右侧颞中回、左侧枕下回。且右岛叶的ReHo值的增加与病程之间呈正相关;右侧颞中回ReHo值的减低与VAS评分之间呈负相关。结论:颈源性头痛累积多个涉及情感、记忆、认知的脑区,是一种复杂的病理状态。Objective:Using the regional homogeneity method of resting brain functional magnetic resonance imaging(fMRI),we investigated the mechanism of cervicogenic headache in the brain.Methods:Resting brain fMRI was performed in 13 patients with cervicogenic headache(CEH group)and 13 healthy control(HC group)with GE 3.0T.The post-processing software REST was used to analyze the data.Correlation analysis was performed between Regional Homogeneity(ReHo)of different brain areas with visual analogue scale(VAS)and disease course.Results:Compared with HC group,the brain areas with increased ReHo value of CEH group were the bilateral precuneus,left corpus callosum,left dorsal thalamus,left parahippocampal gyrus,right insula,right occipital middle gyrus,and bilateral cerebellar hemispheres.Brain regions with decreased ReHo were bilateral superior frontal gyrus-middle frontal gyrus-anterior cingulate gyrus,right central posterior gyrus,right precuneus-posterior cingulate gyrus,right middle temporal gyrus,and left inferior occipital gyrus.In addition,the increase of ReHo value in the right insula was positively correlated with the course of disease;there was a negative correlation between the decrease of ReHo value in the right middle temporal gyrus and the VAS score.Conclusion:Cervicogenic headache is a complex pathological state that affects multiple brain areas,involving emotion,memory,cognitive and other aspects.
