负压伤口治疗辅助逆行腓肠神经营养血管皮瓣修复足踝部创面10例

摘　　要：目的观察负压伤口治疗(NPWT)辅助逆行腓肠神经营养血管皮瓣修复足踝部创面的临床疗效。方法回顾性分析2019年1~12月暨南大学附属广州红十字会医院烧伤整形科收治的10例应用NPWT辅助逆行腓肠神经营养血管皮瓣修复足踝部创面患者的临床资料,皮瓣切取范围6.0 cm×4.0 cm^15.0 cm×7.0 cm。供区直接缝合或植皮修复。结果 NWPT治疗7 d后所有皮瓣成活良好,无红肿、皮肤破溃及坏死。缝合口愈合良好,无感染、出血及裂开。创面修复后一期愈合,供区皮片成活良好。术后平均住院天数为14 d。结论 NPWT辅助逆行腓肠神经营养血管皮瓣修复足踝部创面疗效显著,具有简单、安全、皮瓣成活率高等优点,是一种较理想的治疗方法。Objective To observe the clinical effect of negative pressure wound therapy(NPWT) assisted retrograde sural neurovascular flap for repairing wound of foot and ankle. Methods The clinical data of 10 patients with ankle and foot wound were retrospectively analyzed, who were treated with NPWT-assisted retrograde sural neurovascular flap in the Department of Burn and Plastic Surgery at the Guangzhou Red Cross Hospital Affiliated to Jinan University from January 2019 to December 2019. The sizes of flap was in a range of 6.0 cm×4.0 cm^15.0 cm×7.0 cm. The donor area was repaired with direct suture or skin graft. Results After 7 days of NWPT-assisted treatment, all flaps survived well without redness, skin ulceration, or necrosis. The suture incision healed well without infection, hemorrhage, or dehiscence. The wound was healing by first intention after repair, and the skin graft donor site survived well. The average postoperative hospital stay was 14 days. Conclusion Negative pressure wound therapy assisted retrograde sural neurovascular flap has obvious curative effect, has the advantages of simplicity, safety, and high flap survival rate. It is an ideal treatment method for repairing wound of the ankle and foot.
