摘　　要：目的探讨血清淀粉样蛋白A(SAA)对慢性阻塞性肺疾病(COPD)并心力衰竭(HF)的诊断及预防价值。方法选取2017年7月-2018年7月收治的COPD并HF患者60例,根据患者病情分为AECOPD-HF组及COPD-HF组,每组30例,选择同期进行体检的健康人群30例为健康组。检测3组SAA及血清降钙素原(PCT)水平,并分析其对COPD并HF的诊断效能。结果AECOPD-HF组SAA及PCT水平较COPD-HF、健康组升高,且COPD-HF组高于健康组(P<0.05)。ROC曲线分析显示,SAA与PCT对COPD并HF的诊断效能曲线下面积比较差异无统计学意义(P>0.05),但SAA灵敏度较PCT高(P<0.05)。结论SAA对COPD并HF的诊断灵敏度高,对COPD并HF患者的病情控制有重要意义。Objective To explore the value of serum amyloid A(SAA)in the diagnosis and prevention of chronic obstructive pulmonary disease(COPD)complicated with heart failure(HF).Methods Sixty patients with COPD and HF who were admitted during July 2017 and July 2018 were enrolled and divided into AECOPD-HF group(n=30)and COPD-HF group(n=30)according to the severity of disease.Thirty people who underwent physical examination during the same period were selected as the healthy group.The levels of SAA and procalcitonin(PCT)in the three groups were detected,and their diagnostic efficacy for COPD and HF was analyzed.Results The SAA and PCT levels in the AECOPD-HF group were higher than those in the COPD-HF and healthy groups,and the above indicators in the COPD-HF group was higher than those in the healthy group(P<0.05).ROC curve analysis showed that there was no statistically significant difference in the area under the curve of diagnostic efficacy of SAA and PCT for COPD and HF(P>0.05),but the sensitivity of SAA was higher than that of PCT(P<0.05).Conclusion SAA has a higher diagnostic sensitivity for COPD and HF,and is of great significance to the control of COPD and HF in patients.
