化脓性中耳炎中医证型与纯音听阈结果的关系研究

摘　　要：目的:探讨化脓性中耳炎患者中医证型与纯音听阈测定结果的关系。方法:收集155例化脓性中耳炎患者(248例患耳)的中医辨证资料及纯音听阈测定结果,采用SPSS 20.0软件对所收集数据进行统计分析。结果:1)各证型气导听阈在全频段均有升高。除4 KHz外不同中医证型患耳在0.125~2、8 KHz上气导听阈的差异均有统计学意义(P<0.05);2)除脾虚湿困证外,外邪侵袭证、肝胆湿热证及肾元亏损证3个证型组患耳气导听阈在各频率之间比较差异有统计学意义(P<0.05);3)分别在0.25~4 KHz时不同证型患耳骨导听阈比较差异均有统计学意义(P<0.05);4)肾元亏损证患者骨导听阈全频段升高,其余3种证型骨导听阈在各频率段基本处于正常范围。结论:不同中医证型化脓性中耳炎患者的纯音听阈测定结果表现各异,分析纯音听阈测定结果有助于化脓性中耳炎的中医辨证分型,对疾病的治疗及预后均有一定帮助。Objective:To explore the connections between TCM patterns of suppurative otitis media and the results of pure tone audiometry.Methods:The data of TCM syndrome differentiation and the results of pure tone audiometry of 155 patients(248 ears)were collected,and the data was statistically analyzed using SPSS 20.0 software.Results:1)Air conduction threshold of different patterns rose in the whole frequency band.Except 4KHz,the difference was statistically significant in the air conduction thresholds of different patterns in the frequency band from 0.125 to 2.8 KHz(P<0.05);2)Except spleen deficiency and dampness retention pattern,the difference had statistical meaning in air conduction thresholds of external pathogenic invasion pattern,liver and gallbladder dampness-heat pattern and kidney loss pattern(P<0.05);3)The difference showed statistical meaning in the comparisons of bone conduction thresholds of different patterns in the frequency band from 0.25 to 4 KHz(P<0.05);4)Bone conduction thresholds of the patients of kidney loss pattern increased in the whole frequency band,they were in the normal range in the patients of other three patterns in different frequency bands.Conclusion:The results of pure tone audiometry are various in the patients of different TCM patterns,the analysis of the results could help syndrome differentiation and patterns,the treatment and prognosis of the disease.
