压力梯度长袜联合使用间歇充气装置预防淋巴囊肿的效果观察

摘　　要：目的运用护理干预方法,探讨预防盆腔淋巴清扫术后淋巴囊肿发生的影响。方法选取2016年1月至2016年12月无锡市人民医院诊治的100例行盆腔淋巴结清扫术后的妇科恶性肿瘤住院患者作为研究对象。根据随机数字表法随机分为试验组(n=50)和对照组(n=50),试验组运采用压力梯度长袜(CS)联合使用间歇充气装置(IPC),对照组运用常规方法。比较两组患者盆腔清扫手术术中情况(清扫淋巴结数目、术中出血量、手术时长)、术后恢复情况(白蛋白值、引流量、留置尿管时长、住院时间)、淋巴囊肿发生率、淋巴囊肿的大小和患者住院满意度。结果两组患者手术术中清扫淋巴结数目、出血量、手术时长、患者住院时间差异均无统计学意义(P>0.05)。试验组盆腔淋巴清扫术后、引流量、留置尿管时长,淋巴囊肿发生率、淋巴囊肿的大小均低于对照组,白蛋白值和患者的住院满意度均高于对照组,差异具有统计学意义(P<0.05)。结论压力梯度长袜(CS)联合使用间歇充气装置(IPC)这一护理干预方法能降低盆腔淋巴清扫手术术后淋巴囊肿的发生率。Objective To investigate the effect of nursing intervention on preventing the occurrence of lymphatic cyst after pelvic lymphadenectomy.Methods 100 patients with gynecological malignant tumor who underwent pelvic lymph node dissection in our hospital from January 2016 to December 2016 were selected.The patients were randomly divided into experimental group(n=50)and control group(n=50)according to random digital table method.Patients in the control group received routine treatment and patients in the experimental group received graduated compression stocking(CS)combined with intermittent pneumatic compression(IPC).The intraoperative situation(the number of lymph nodes,intraoperative blood loss,the length of operation),postoperative recovery(albumin value,drainage volume,indwelling catheter duration,hospitalization time),the incidence of lymphoid cysts,the size of lymphoid cysts and the satisfaction of hospitalization were compared between the two groups.Results There was no statistically significant difference in the number of lymph nodes,the amount of bleeding,the length of operation and the hospitalization time between the two groups(P>0.05).The drainage rate,indwelling catheter time,the incidence of lymphoid cyst and the size of lymphoid cyst in the experimental group were lower than those in the control group,and the albumin value and hospitalization satisfaction of the patients in the experimental group were significantly higher than those in the control group,with statistically significant differences(P<0.05).Conclusions Nursing intervention of CS combined with IPC can effectively reduce the incidence of lymphatic cyst after pelvic lymphadenectomy.
