摘 要:目的回顾性分析新型冠状病毒肺炎(COVID-19)患者入院时外周血白细胞、中性粒细胞及T淋巴细胞亚群的临床意义。方法回顾性收集256例火神山医院住院确诊为COVID-19的患者,其中轻型、普通型172例,重型、危重型84例。检测外周血T淋巴细胞亚群,测定其白细胞计数及中性粒细胞计数等,同时收集患者基本信息,包括发病时间、抽血时间等。结果 (1)重型、危重型患者年龄(62.74±14.94)岁,轻型、普通型患者年龄(57.63±13.35)岁,两组年龄差异有统计学意义(P<0.05)。(2)重型、危重型患者白细胞计数、中性粒细胞计数较轻型、普通型患者明显增多,差异有统计学意义(P<0.05)。(3)两者淋巴细胞总数、CD3^+、CD3^+CD4^+、CD3^+CD8^+淋巴细胞计数均低于正常范围,而重、型危重型患者更为低下,差异有统计学意义(P<0.05)。(4)重型、危重型患者NK细胞低于正常范围,而与轻型普通型患者相比,差异有统计学意义(P<0.05)。(5)相关性分析显示,发病至抽血间隔时间与白细胞以及中性粒细胞计数呈负相关,差异有统计学意义(P<0.05)。结论 COVID-19患者外周血淋巴细胞、白细胞、中性粒细胞检测有助于COVID-19患者的筛查、诊断和治疗。Objective This study retrospectively analyzed the clinical significance of white blood cells,neu⁃trophils and T lymphocyte subsets in peripheral blood of patients with COVID⁃19 on admission.Methods A total of 256 patients with new coronavirus pneumonia diagnosed in Huoshenshan Hospital were collected retrospectively,which included mild common type(n=172)and severe type(n=84).Peripheral blood T lymphocyte subsets,leukocyte count and neutrophil count were measured.At the same time,the basic information of patients was collected,wihch included the time of onset,the time of drawing blood and so on.Results(1)The age of severe patients was(62.74±14.94)years old,and that of mild common patients was(57.63±13.35)years old.The age of the former increased significantly(P<0.05).(2)The leukocyte counts and neutrophil counts in severe patients were significantly higher than those in mild patients(P<0.05).(3)The total number of lymphocytes,CD3^+,CD3^+CD4^+,CD3^+CD8^+lymphocyte counts of the two were lower than the normal range,while severe and critical patients are even lower and the difference was statistically significant(P<0.05).(4)The number of NK cells in severe patients was lower than that in normal patients,and there was significant difference between severe patients and mild patients(P<0.05).(5)Correlation analysis showed that the interval between onset and blood draw were neg⁃atively correlated with white blood cells and neutrophils and the difference was statistically significant(P<0.05).Conclusion The detection of peripheral blood lymphocytes,leukocytes and neutrophils in patients with COVID⁃19 was helpful for the screening,diagnosis and treatment of patients with COVID⁃19.
