摘 要:目的分析血胆碱酯酶活力(CHE)联合心肌酶、B型利尿钠肽(BNP)检测在急性有机磷农药中毒心肌损害诊断中的应用价值。方法将2016年6月至2019年6月本院收治的有机磷农药中毒心肌损害患者43例作为观察组,选择同期于本院行常规检查的健康对象43名作为对照组。比较2组CHE、心肌酶及BNP水平,并研究观察组不同中毒程度患者的水平。结果观察组轻度、中度、重度中毒患者CHE活性均低于对照组,且中毒越深患者CHE活性越低;观察组第1~7天心肌酶及BNP水平均高于对照组,其中以第3天最为显著;观察组轻度、中度、重度中毒患者心肌酶水平均高于对照组,且中毒程度越深心肌酶水平越高,差异具有统计学意义(P<0.05)。结论CHE联合心肌酶、BNP检测,能够对有机磷农药中毒心肌损害患者作出准确诊断,以此来指导临床用药以及对患者预后作出评价,进而降低患者的病死率、并发症发生率。Objective To analyze the application value of blood cholinesterase activity(CHE)combined with myocardial enzyme and B-type natriuretic peptide(BNP)detection in the diagnosis of myocardial damage caused by acute organophosphorus pesticide poisoning.Methods A total of 43 patients with myocardial damage caused by organophosphorus pesticide poisoning admitted to our hospital from June 2016 to June 2019 were enrolled in the study.A total of 43 healthy subjects who underwent routine examination in our hospital were selected as the control group.The levels of blood CHE,myocardial enzymes and BNP were compared between the two groups,and the differences in the levels of patients with different degrees of poisoning were observed.Results The CHE activity of patients with mild,moderate and severe poisoning in the observation group was lower than that of the control group,and the CHE activity was lower in patients with deeper poisoning.The myocardial enzymes and BNP levels in the observation group were higher than those in the control group on the 1 st to 7 th day.The difference of 3 d was the most significant.The level of myocardial enzymes in the mild,moderate and severe poisoning patients in the observation group was significantly higher than that in the control group,and the deeper the degree of poisoning,the higher the myocardial enzyme level,the difference was statistically significant(P<0.05).Conclusion Combined with blood CHE combined with myocardial enzyme and BNP detection,it can accurately diagnose patients with myocardial damage caused by organophosphorus pesticide poisoning,in order to guide clinical medication and evaluate the prognosis of patients,thereby reducing the mortality and complications of patients.
