董氏奇穴结合耳尖放血治疗肝郁化火型失眠的疗效观察

摘　　要：目的:观察董氏奇穴结合耳尖放血治疗肝郁化火型失眠的临床疗效及对5-羟色胺(5-hydroxytryptamine,5-HT)、褪黑素(Melatonine,MT)水平的影响。方法:将100例肝郁化火型失眠患者随机分为2组,每组50例,对照组予耳尖放血治疗,观察组予董氏奇穴结合耳尖放血治疗,治疗4周。观察2组患者治疗前后匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quality Index,PSQI)评分、睡眠状况自评量表(Self-Rating Scale of Sleep,SRSS)评分、多导睡眠监测(PSG)指标、中医证候评分及5-HT、MT水平。结果:观察组治疗总有效率显著高于对照组(P <0. 05)。2组组内比较发现治疗后PSQI评分、SRSS评分、中医证候量表评分均较治疗前明显降低(P <0. 05),观察组治疗后评分显著低于对照组(P <0. 05)。2组组内比较发现治疗后睡眠潜伏期较治疗前明显缩短(P <0. 05),总睡眠时间较治疗前明显延长(P <0. 05),睡眠效率较治疗前明显提高(P <0. 05),观察组治疗后较对照组睡眠潜伏期缩短更为显著(P <0. 05),总睡眠时间延长更为显著(P <0. 05),睡眠效率提高更为显著(P <0. 05)。对照组治疗前后5-HT、MT水平比较差异无统计学意义(P> 0. 05),观察组治疗后5-HT、MT水平较治疗前均明显升高(P <0. 05)。结论:董氏奇穴结合耳尖放血治疗能够显著延长肝郁化火型失眠患者睡眠时间、增加睡眠深度、促进入睡,并有助于改善周围症状,发挥疗效的机制可能与有效调节5-HT、MT水平有关。Objective:To observe the clinical effects of Dong′s Extra Acupoints combined with pinna tip bloodletting on insomnia of liver depression transforming into fire,and its influence on 5-hydroxytryptamine(5-HT)and melatonine(MT).Methods:One hundred patients with insomnia of liver depression transforming into fire were randomly divided into 2 groups.Fifty patients in the control group were given pinna tip bloodletting.Fifty patients in the observation group were treated with Dong′s Extra Acupoints on the basis.Both groups were treated for 2 courses,2 weeks a course.The scores of PSQI,Self-rating Scale of Sleep(SRSS),polysomnography monitor indexes,TCM syndrome scores,and the levels of 5-HT and MT before and after treatment were compared,and the clinical efficacy were assessed in the two groups.Results:The total effective rate of the observation group was more efficient than that of the control group(P<0.05).Both of the two groups,after treatment,the scores of PSQI,SRSS,sleep latency,and TCM syndrome were lower than before treatment,and scores of the observation group were more lower than those of the control group after treatment(P<0.05).While sleep duration and sleep efficiency were better than those before treatment in both of the two groups,and more higher in the observation group after treatment(P<0.05).The levels of 5-HT and MT have no significant change in control group after and before treatment.After treatment,their levels were higher than those before treatment in observation group.Conclusion:Dong′s Extra Acupoints combined with pinna tip bloodletting on insomnia of liver depression forming fire can significantly prolong the sleep duration,increase sleep depth,shorten sleep latency,and help to alleviate the associated symptoms in patients with insomnia.The mechanism may be related with its effective regulation of the levels of 5-HT and MT.
