经鼻高流量湿化治疗AECOPD合并慢性呼吸衰竭患者的效果研究

摘　　要：目的探讨在常规治疗和护理基础上加用经鼻高流量湿化治疗对AECOPD合并慢性呼吸衰竭患者护理状况的改善情况。方法采用自身对照研究,选取广州市某三甲医院2016年7月—2017年12月入院的74例AECOPD合并慢性呼吸衰竭患者为研究对象,在常规治疗和护理的基础上接受经鼻高流量湿化治疗(HFNC),比较使用前后患者PaO2、PaCO2、痰液黏稠度改善情况、患者舒适度及腹胀、颜面部压迫等护理相关不良反应的发生率。结果患者采用经鼻高流量湿化的平均时间为(186.27±138.651)h;与使用前相比,患者使用后PaO2、PaCO2、痰液黏稠度、舒适度总分、生理维度、心理维度和社会文化维度得分均优于使用前,差异有统计学意义(P<0.05);患者治疗过程中腹胀和颜面部压迫发生率分别为0、1.35%。结论采用经鼻高流量湿化治疗可为AECOPD合并慢性呼吸衰竭患者提供一种新的无创通气护理模式,能有效改善患者氧合状况,促进患者痰液稀释,提高患者舒适度。Objective Discussed the improving situation on patients with acute exacerbation chronic obstructive pulmonary disease(AECOPD)combined with chronic respiratory failure by using high flow nasal cannula(HFNC)on the basis of routine treatment and nursing.Methods Totally 74 patients with AECOPD combined with chronic respiratory failure in Guangzhou were recruited from July 2016 to December 2017.On the basis of routine treatment and nursing,HFNC is treated.Compared before and after use the patient′s PaO2,PaCO2,sputum viscosity improvement,comfort and treatment-related adverse reactions,such as abdominal distention,facial oppression after intervention.Results The average time of the patients with HFNC was(186.27±138.651)h,and the differences between PaO2,PaCO2,sputum viscosity,the scores of the total score,physiological dimension,psychological dimension and socio-cultural dimension were statistically significant(P<0.05)compared with before use.The incidence of abdominal distension and facial compression was 0 and 1.35%respectively.Conclusion HFNC provide a new kind of noninvasive ventilation for clinical nursing care model and can effectively improve patient′s oxygenation,sputum diluted,comfort,and can improve the nursing level.
