金元医家从脾胃论治眩晕的思路、治法及其对后世的影响

摘　　要：对金元医家从脾胃论治眩晕的思路、治法及其对后世医家的影响进行了分析。金元医家丰富了眩晕的病因、病机及治法,其中刘完素倡风火致眩而重脾胃润养,李东垣主升降相因而重脾胃元气,朱丹溪推痰火致眩而重湿土生痰,张子和尚痰实致眩而重攻邪护胃,后世医家基于四者治疗眩晕的思路与治法,从培土荣肝、补中调气、健脾祛痰、祛邪护胃等角度进行治疗,拓展了眩晕的治法,亦为当代医家从脾胃论治眩晕提供了思路。This paper analyses the thinking,therapeutic method of doctors in Jin and Yuan dynasties in differential treatment of vertigo from the spleen and stomach and its influence on later generations.Doctors in Jin and Yuan dynasties have enriched the etiology,pathogenesis and therapeutic method of vertigo,among whom LIU Wansu advocated wind-fire resulting in vertigo,thus emphasizing moistening and nourishing spleen and stomach;LI Dongyuan deemed mutual causality between ascending and descending,thus focusing on primordial qi in spleen and stomach;ZHU Danxi held phlegm-fire leading to vertigo,thus stressing damp-earth producing phlegm;ZHANG Zihe believed sthenia-phlegm causing vertigo,thus valuing eliminating pathogenic factors and protecting stomach.Based on the thinking and therapeutic method of treating vertigo by the four doctors,the doctors of the later generations treated vertigo from the perspectives of banking up earth to nourish the liver,strengthening middle energizer to regulate qi,invigorating spleen to expel phlegm,expelling pathogenic factors to protect the stomach,etc.,which expanded the therapeutic method of vertigo and also provided thinking for the contemporary doctors to differentially treat vertigo from the spleen and stomach.
