新安医家孙一奎《赤水玄珠》辨治脚气病药物配伍特色探析

摘　　要：通过整理新安医家孙一奎《赤水玄珠》中治疗脚气病的57个效方,提炼孙氏治疗脚气病的核心药对,分析其辨治脚气病的药物配伍特色。其认为脚气病的发病有风、寒、湿、热、虚等因素,诸邪常合而致病,且易滞血成瘀,兼见大便不通、小便赤涩,更有脚气入腹、冲心及迫肺之变证,并根据病情变化分别确立了治疗策略。另外,孙氏善于将专效的核心药对灵活配伍,对脚气病的不同证型和不同时期病情变化起到针对性治疗,拓展了该病的临床治疗方法。By collating 57 efficacious prescriptions for treating beriberi in Chishui Xuanzhu by SUN Yikui,a physician in Xin’an,this paper extracts the core couplet medicinals of SUN’s in the treatment of beriberi,and analyses the characteristics of medicine compatibility of his differential treatment for beriberi.He holds that the pathogenic factors of beriberi lie in wind,cold,dampness,heat and asthenia,all of which are often combined to cause disease,easily giving rise to blood stasis.There can also be constipation,hot and difficult urination,moreover,the deteriorated syndrome of beriberi entering abdomen,attacking heart and affecting lung.He establishes a treatment strategy according to the changes in patients’conditions.In addition,SUN’s excels at flexible compatibility of the core couplet medicinals with special effect,and has targeted treatment for different syndrome types and disease changes in different periods of beriberi,and expanded the clinical therapeutic methods of it.
