摘  要:目的探讨痰液及血清中单核细胞趋化因子蛋白(MCP-1)、白介素-35表达在肺结核患者检测中的临床意义。方法回顾性分析本院收治的106例PT患者(疾病组)、35例肺疾病患者(对照组)的临床资料,依据疾病组病情程度分为疾病活动组(活动性PT,n=63)、疾病稳定组(稳定期PT,n=43),比较各组痰液细胞和血清中MCP-1及IL-35水平,比较不同性别、年龄的PT患者痰液细胞和血清中MCP-1及IL-35水平,并分析PT病情程度与痰液细胞和血清中MCP-1、IL-35水平的关系。结果疾病组痰液细胞、血清中MCP-1表达水平高于对照组,IL-35表达水平低于对照组(P<0.001),且疾病组痰液细胞中MCP-1及IL-35表达水平均高于血清中的水平(P<0.001);不同年龄、性别的PT患者痰液细胞、血清中MCP-1及IL-35表达水平比较,差异无统计学意义(P>0.05);疾病活动组痰液细胞、血清中MCP-1水平高于疾病稳定组,IL-35表达水平低于疾病稳定组(P<0.001);PT患者病情程度与痰液细胞、血清中MCP-1水平呈正相关(P<0.001),与IL-35水平呈负相关(P<0.001);痰液及血清中MCP-1、IL-35联合检测诊断PT病情程度的灵敏度和准确度高于MCP-1、IL-35各自检测诊断的灵敏度和准确度。结论PT患者痰液细胞、血清中MCP-1水平明显升高而IL-35水平明显降低,痰液细胞、血清中MCP-1、IL-35水平或可有效预测PT的病情程度。Objective To explore the clinical significance of MCP-1 and IL-35 in sputum and serum in the detection of pulmonary tuberculosis(PT).Methods Clinical data of 106 patients with PT(disease group)and 35 patients with other lung diseases(control group)were retrospectively analyzed.Patients in the disease group were divided into the active group(active PT,n=63)and the stable group(stable PT,n=43)according to the condition of the disease.The levels of MCP-1 and IL-35 in sputum and serum were compared between groups and between patients with PT of different gender and age.The relationship between the severity of PT and levels of MCP-1 and IL-35 in sputum and serum was analyzed.Results The expression levels of MCP-1 in sputum and serum of the disease group were significantly higher than those in the control group while the expression levels of IL-35 were significantly lower than that in the control group(P<0.001).The levels of MCP-1 and IL-35 in sputum of the disease group were significantly higher than those in serum(P<0.001).There was no significant difference in the expression level of MCP-1 or IL-35 in sputum or serum between patients with PT of different ages or genders(P>0.05).The levels of MCP-1 in sputum and serum of the active group were significantly higher than those in the stable group while the levels of IL-35 in sputum and serum of the active group were significantly lower than those in the stable group(P<0.001).The severity of disease in the PT patients was positively correlated with MCP-1 levels in sputum and serum(P<0.001)while negatively correlated with IL35(P<0.001).The sensitivity and accuracy of combined detection of MCP-1 and IL-35 in sputum and serum for PT diseases conditions were significantly higher than those of serum MCP-1 and IL-35 detected alone.Conclusion MCP-1 levels in sputum and serum of patients with PT are significantly increased while IL-35 levels are significantly decreased.The levels of MCP-1 and IL-35 in sputum cells and serum can effectively predict the severity of PT.
