摘　　要：目的探讨电子计算机断层扫描血管造影(CTA)与脊柱3D重建联合对脊柱侧弯畸形术前评估和手术计划制定的临床价值。方法特发性脊柱侧弯患者116例,均进行CTA检测并进行脊柱3D重建,并以影像学结果指导实施脊柱侧弯畸形术。统计不同CT后处理方式对脊柱侧弯患者病变显示的满意程度,分析术前评估椎体与主动脉安全性与手术结果一致性,并测量手术前后主弯角度、后凸角度、Cobb角。结果CTA+3D脊柱重建处理方式对脊柱侧弯患者病变显示情况更好,满意度高于其他处理方式;术前评估椎体与主动脉安全性与手术结果无明显差异(P>0.05);手术后患者主弯角度、后凸角度、Cobb角均小于手术前(P<0.05)。结论CTA与脊柱3D重建联合检测脊柱侧弯畸形术前患者可评估手术安全性。Objective To investigate the clinical value of computed tomography angiography(CTA)combined with spinal 3D reconstruction in preoperative evaluation and surgical plan development for scoliosis.Methods A total of 116 patients with idiopathic scoliosis treated in our hospitals received CTA examination and spinal 3D reconstructions were selected.Based on the imaging results,scoliosis surgery was performed.The display of lesions in patients with scoliosis by different CT post-processing methods was statistically analyzed.The consistency between the preoperative evaluation of vertebral body and aorta and the surgical results was analyzed.The main bending angle,kyphosis angle and Cobb angle were measured before and after surgery.Results Lesions in patients with scoliosis were shown better by using CTA combined with spinal 3D reconstruction,and the satisfaction was higher than other methods.There was no significant difference between the preoperative evaluation of vertebral body and aorta and the surgical results(P>0.05).After surgery,the main bending angle,kyphosis angle and Cobb angle of patients were smaller than those before surgery(P<0.05).Conclusion CTA combined with spinal 3D reconstruction can be used to evaluate the safety of surgery for patients with scoliosis.
