低心率和高心率冠状动脉血管CT造影的最佳图像成像期相探讨

摘　　要：目的探讨低心率和高心率冠状动脉血管CT造影图像质量最佳的R-R间期期相。方法选取2017年6月至11月在我院行冠状动脉血管CT造影检查的60例患者,分为低心率组(HR≤70次/分)和高心率组(HR>70次/分)各30例;在20%~80%的R-R间期曝光,每隔5%间隔进行重建,对不同R-R期相的冠状动脉图像质量进行评分,测定低心率和高心率组图像质量最佳期相以及与最佳期相无统计学差异的期相。结果低心率组中右冠状动脉(RCA)、左主干(LM)、前降支(LAD)、回旋支(Cx)图像质量最佳期相均位于75%R-R期相。高心率组中RCA的最佳期相在45%R-R间期,LM最佳期相在40%R-R间期,LAD、Cx的最佳期在45%R-R间期;与最佳期相无统计学差异的期相RCA在40%R-R间期,LM在35%和50%R-R间期,LAD在35%和40%R-R间期,Cx在40%R-R间期(P<0.05)。结论HR≤70次/分的冠状动脉CTA最佳曝光期相范围在65%~75%R-R间期;HR>70次/分的冠状动脉血管CT造影最佳曝光期相在35%~45%R-R间期。Objective To explore the best R-R interval phase for CTA image quality of coronary artery at low and high heart rate.Methods From June 2017 to November 2017,60 patients received CTA examination were divided into low heart rate(≤70 beats/min)group and high heart rate group(>70 beats/min),30 in each group.Exposure was performed between 20%and 80%R-R interval phase.Reconstruction was carried out every 5%interval.CTA image quality of coronary artery at different R-R interval phases was evaluated.The best phase of image quality and the phase with no statistical difference from the best phase at the low heart rate and high heart rate were measured.Results In the low heart rate group,the best phase of RCA,LM,LAD and CX image quality was 75%R-R interval.In the high heart rate group,the best phase of RCA image quality was 45%R-R interval,LM was 40%R-R interval,LM was 35%to 50%R-R interval,and LAD and Cx were 45%R-R interval.In the phase with no statistical difference from the best phase,RCA was 40%R-R interval,LM was 35%to 50%R-R interval,LAD was 35%to 40%interval and Cx was 40%R-R interval(P<0.05).Conclusion When the patients with heart rate≤70 beats/min,the best image quality is obtained in the range of 65%-75%of R-R interval.When the patients with heart rate>70 beats/min,the best image quality is obtained in the range of 35%-45%of R-R interval.
