皮下注射纳米脂肪联合泪槽凹陷区域注射颗粒脂肪对下睑混合型黑眼圈患者皮肤色泽的影响

摘　　要：目的观察皮下注射纳米脂肪联合泪槽凹陷区域注射颗粒脂肪对下睑混合型黑眼圈患者皮肤色泽的影响。方法郑州大学第五附属医院接受治疗的102例下睑混合型黑眼圈患者,按随机数字表法分为观察组与对照组各51例。对照组给予泪槽凹陷区域注射颗粒脂肪治疗,观察组在其基础上联合皮下注射纳米脂肪进行治疗。比较两组短期(3个月内)临床疗效,并记录其术前、术后6个月的皮肤色素沉积情况红斑指数(EI)、黑素指数(MI)、皮肤色泽指标亮度(L^*值)、红/绿度(a^*值)、黄/蓝度(b^*值)变化及术后效果满意度视觉模拟评分法(VAS)与随访过程中相关并发症发生情况。结果治疗3个月后,观察组总有效率高于对照组(P<0.05);术后6个月,两组EI、MI较治疗前下降,且观察组低于对照组;两组L^*值较治疗前上升,且观察组高于对照组(P<0.05),两组a^*值、b^*值治疗前后比较差异均无统计学意义(P>0.05);观察组效果满意率高于对照组(P<0.05),两组临界及效果不满意等级比较差异均无统计学意义(P>0.05);两组红肿、感染、脂肪结节、脂肪坏死、皮下瘀斑发生率比较差异均无统计学意义(P>0.05)。结论皮下注射纳米脂肪联合泪槽凹陷区域注射颗粒脂肪确可有效改善下睑混合型黑眼圈患者临床症状,提高其临床疗效,且还有利于皮肤色泽的改善。Objective To observe the effects of subcutaneous injection of nano-fat combined with injection of fat granule in lacrimal groove depression area on the skin color of patients with mixed lower eyelid dark circles.Methods A total of 102 patients with mixed lower eyelid dark circles treated in our hospital were selected as subjects.They were divided into observation or control group according to the random number table method,51 in each group.The control group was given an injection of fat granule in lacrimal groove depression area,and the observation group was combined with subcutaneous injection of nano-fat.The short-term(within 3 months)clinical efficacy was compared between the two groups,and the skin pigmentation such as erythema index(EI),and melanin index(MI)and skin color indexes such as brightness(L^*value),red/green degree(a^*value),yellow/blue degree(b^*value)before surgery and after 6 months of surgery,and postoperative effect satisfaction such as visual analogue scale(VAS)and occurrence of related complications during follow-up were recorded.Results After 3 months of treatment,the total effective rate in the observation group was higher than that in the control group(P<0.05).After 6 months of surgery,the EI and MI in the two groups were significantly decreased compared with those before treatment(P<0.05),and the indexes in the observation group were significantly lower than those in the control group(P<0.05).After 6 months of surgery,the L^*values in the two groups were significantly increased compared with those before treatment(P<0.05),and the value in the observation group was significantly higher than that in the control group(P<0.05).There were no statistically significant differences in the a^*value and b^*value between the two groups before and after treatment(P>0.05).After 6 months of surgery,the effect satisfaction rate in the observation group was higher than that in the control group(P<0.05).There were no statistically significant differences in the critical value and effect dissatisfaction g
