摘　　要：目的分析及评估采用保留肛门括约肌挂线术术式治疗高位经括约肌肛瘘的临床效果。方法选取2017年1月至2019年6月江阴市人民医院肛肠外科收治的40例高位经括约肌肛瘘患者,随机分为实验组与对照组,每组20例。实验组采用保留括约肌挂线术,对照组采用切割挂线术,对比两组临床效果。结果实验组患者住院时间、完全愈合时间长于对照组(P<0.01),但术后疼痛评分及肛门失禁评分低于对照组(P<0.01);两组患者治愈率及复发率比较差异无统计学意义。结论保留括约肌挂线治疗高位经括约肌肛瘘具有可靠的临床效果,不仅可以治愈患者的疾病,而且可以相对减轻患者术后疼痛、保护患者的肛门功能,并拥有与肛瘘切割挂线术类似的治愈率,虽住院时间、完全愈合时间相对较长,但其优势更加适用于临床。Objective To analyze and evaluate the clinical effect of loose setonin the surgical treatment for high transsphincter fistula. Methods 40 patients with high transsphincter fistula admitted to the anorectal surgery department of Jiangyin people’ s hospital from January 2017 to June 2019 were selected, they were randomly divided into the experimental group who were treated with loose seton and the control group who were treated with cut seton, 20 patients in each group, to compare the clinical effects of the two groups. Results The length of hospital stay and complete healing time of the experimental group were longer than the control group(P<0.01), while the scores of postoperative pain and the anal incontinence were obviously below than the control group(P<0.01). There was no significant difference in cure rate and the recurrence between the two groups.Conclusion The loose seton in the surgical treatment for high transsphincter fistula has reliable curative effect. When it removal of lesions, not only can cure patients’ diseases, but also can reduce postoperative pain and protect the anal function. Compare with the two groups, have the similar cure rate and the recurrence. Although the length of hospital stay and complete healing time of the loose steon is relatively long, but the advantages more practical in the clinic.
