摘　　要：目的探讨不同浓度罗哌卡因复合舒芬太尼硬膜外阻滞在产程潜伏期阶梯式分娩镇痛中的效果。方法选择2017年2月至2019年5月在本院分娩的70名产妇,随机分为7组,每组10名。每组给予不同浓度罗哌卡因、舒芬太尼治疗,观察比较各组VAS评分、产程、Bromage评分。结果 7组产妇各个时间点VAS评分比较差异无统计学意义;潜伏期B组和C组产程明显低于A组,E组、F组产程显著低于D组,活跃期D组产程较A组明显延长(P<0.05);B组和C组Bromage评分同A组比较明显下降,E组、F组、G组Bromage评分较D组显著降低(P<0.05)。结论针对硬膜外阻滞产程潜伏期产妇应用罗哌卡因0.075%或0.1%联合舒芬太尼0.5μg/mL,活跃期给予罗哌卡因浓度0.125%联合舒芬太尼0.5μg/mL,镇痛效果显著,不影响产妇正常活动,对产程影响小,安全性高,值得临床推广应用。Objective To investigate the effect of different concentrations of ropivacaine combined with sufentanil epidural block on stepwise labor analgesia during labor latency. Methods 70 maternal women who delivered in our hospital from February 2017 to May 2019 were randomly divided into 7 groups, with 10 cases in each group. Each group was treated with different concentrations of ropivacaine and sufentanil. The VAS scores and labor stages Bromage score of each group were observed and compared. Results There was no significant difference in the VAS scores among the 7 groups of mothers at various time points. The incubation period of group B and C was significantly lower than that of group A, and the group E and F were significantly lower than those of group D. The active period of group D was significantly more than that of group A(P<0.05). Bromage scores of groups B and C were significantly lower than those of group A, and Bromage scores of group E, F and G were significantly lower than those of group D(P<00.05). Conclusion Lapivacaine 0.075% or 0.1% combined with sufentanil 0.5 μg/mL for laparopausal women during the incubation period of epidural block, and 0.125% ropivacaine combined with sufentanil 0.5 μg/mL during active phase. The analgesic effect is significant, does not affect the normal maternal activities, has small impact on the labor process, high safety, and is worth promoting and applying.
