摘  要:目的分析荧光定量PCR检测法联合传统涂片抗酸染色法、结核抗体检测法以及结核杆菌培养法在结核杆菌检测中的作用。方法选取本院2016年1月至2019年1月经临床确诊为结核病患者120例,依据结核部位分为肺结核组和肺外结核组,各60例,分别进行荧光定量PCR检测法、抗酸染色法、结核抗体检测法以及结核杆菌培养法检测结核杆菌,分析4种方法在初次诊断时的的差异。评价荧光定量PCR检测法联合传统涂片抗酸染色法、结核抗体检测法以及结核杆菌培养法对结核病诊断的影响。结果两组检测结果显示,荧光定量PCR检测法检出率最高,均达到80.0%以上,两组比较差异无统计学意义;结核抗体检测法和结核杆菌培养法组间比较差异无统计学意义;涂片抗酸染色法组间比较差异有统计学意义(P<0.05);4种检测方法单独及联合检测阳性率比较发现,联合检测时阳性率大大提高,灵敏度、特异性、准确性均最高,分别为85.8%、65.3%及86.01%。结论荧光定量PCR检测法联合传统检测法,提高了结核杆菌检测的准确度,值得临床推广应用。Objective To analyze the role of fluorescence quantitative PCR detection combined with traditional smear acid-fast staining, tuberculosis antibody assay and Mycobacterium tuberculosis culture in the detection of Mycobacterium tuberculosis. Methods 120 patients with clinically diagnosed tuberculosis from January 2016 to January 2019 were enrolled. They were divided into tuberculosis group and extrapulmonary tuberculosis group according to the tuberculosis site, with 60 cases in each group. The difference in the detection methods of these four Mycobacterium tuberculosis at the initial diagnosis were analyzed. To evaluate the effect of fluorescence quantitative PCR assay combined with traditional smear acidfast staining, tuberculosis antibody assay and tuberculosis culture on tuberculosis diagnosis. Results The detection results of the two groups showed that the detection rate of the real-time PCR assay was the highest, reaching 80.0% or more. There was no significant difference between the two groups, and the tuberculosis antibody test method and the tuberculosis culture method were also used. There was no statistical significant difference. There was significant difference between the two groups(P<0.05). The comparison of the positive rate of the four detection methods alone and in combination showed that the positive rate was greatly improved, and the highest sensitivity, specificity and accuracy were 85.8%, 65.3% and86.01%, respectively. Conclusion Fluorescence quantitative PCR detection combined with traditional detection method can improve the accuracy of detection of Mycobacterium tuberculosis, which is worthy of clinical application.
