摘 要:目的观察清开灵注射液对脑缺血大鼠急性期病理形态学、炎症因子及凋亡基因Bcl-2和p53表达的影响。方法将60只SD大鼠随机分为假手术组、模型组、阳性组以及实验组。除假手术组外均建立脑缺血大鼠模型。假手术组和模型组给予0.9%氯化钠注射液灌胃,阳性组给予尼莫地平灌胃,实验组注射清开灵注射液,7 d后评估各组大鼠的神经功能评分、神经行为学评分,比较脑梗死体积和神经细胞凋亡。Elisa法检测血清肿瘤坏死因子-α(TNF-α)和白介素-1β(IL-1β)、白介素-6(IL-6)以及白介素-18(IL-18)等炎症因子水平,Western blotting法检测凋亡基因Bcl-2表达,免疫组织化学法检测凋亡基因p53的表达。结果与假手术组比较,模型组大鼠的脑组织神经元明显减少,结构模糊构型紊乱,不规则出现不同程度变性坏死及炎细胞浸润、核体固缩深染,脑梗死体积和神经细胞凋亡明显升高,BBB评分和神经功能评分明显降低(P<0.05);TNF-α、IL-1β、IL-6以及IL-18等炎症因子水平明显升高(P<0.05);Bcl-2表达显著下调,p53表达显著上调(P<0.05)。给予药物干预后,阳性组和实验组的神经元均明显增多,有不同程度的缓解恢复,核固缩核体均有不规则程度的减轻,间质水肿减轻,细胞结构较完整,脑梗死体积和神经细胞凋亡水平明显降低,BBB评分和神经功能评分明显高于模型组(P<0.05),炎症因子明显降低,Bcl-2蛋白水平显著上调,p53表达显著下调(P<0.05)。结论清开灵注射液对脑缺血模型大鼠可有较地减少脑梗死面积,保护脑组织免受炎症因子的损伤,调整凋亡相关因子表达,减少神经细胞的凋亡。Objective:To investigate the effects of Qingkailing Injection on pathological morphology,inflammatory factors and expressions of apoptosis genes Bcl-2 and p53 in the acute phase of cerebral ischemia model rats.Methods:60 SD rats were randomly divided into the sham operation group,the model group,the positive group and the experimental group. Except the sham operation group,rat models of cerebral ischemia were established.The sham operation group and the model group were administered with saline,the positive group with Nimodipine,and the experimental group with Qingkailing Injection. After 7 days,the neurological scores and neurobehavioral scores were evaluated,and the cerebral infarction volume and nerve cell apoptosis were compared. Elisa method was used to detec TNF-α,IL-1β,IL-6 and IL-18. Western blotting was used to detect the expression of apoptotic gene Bcl-2,and immunohistochemical method was used to detect the expression of apoptotic gene p53. Results:Compared with the sham operated group,the neurons in the brain tissue of the model group were significantly reduced;the structure was fuzzy and disordered,and it was irregular with varying degrees of necrosis,infiltration of inflammatory cells and nucleosome pyknosis and hyperchromatism. The volume of cerebral infarction and neuron apoptosis were significantly increased,BBB score and nerve function score were significantly decreased(P< 0.05).The levels of TNF-α,IL-1β,IL-6 and IL-18 were significantly increased(P< 0.05);Bcl-2 expression was significantly down-regulated,p53 expression up-regulated(P< 0.05). After drug intervention,neurons in the positive group and the experimental group were significantly increased,with different degrees of remission and recovery.The nuclear atrophy and nuclear bodies were alleviated with irregular degrees;the interstitial edema was reduced;the cell structure was more complete,and the cerebral infarction volume and neural cell apoptosis levels were significantly reduced;BBB score and neural function score were significantl
