摘  要:目的研究特发性肺动脉高压(IPAH)患者血浆中的miRNA-548a-5p预测血流动力学恶化的价值。方法回顾性纳入2010年5月至2017年6月在上海市肺科医院住院的12例IPAH患者及12名对照者。所有患者均进行了右心导管检查。通过miRNA微阵列基因芯片分析患者和正常对照差异表达的miRNA。线性回归分析miRNA-548a-5p对血流动力学相关参数升高的预测作用。受试者工作特征曲线分析miRNA-548a-5p鉴别患者和对照人群效能。结果IPAH患者和对照人群的一般情况差异无统计学意义。IPAH组miRNA-548a-5p表达明显高于对照组(t=3.038,P<0.05)。部分血流动力学参数和miRNA-548a-5p呈良好的相关性。多重线性回归分析提示miRNA-548a-5p表达水平是预测IPAH患者平均肺动脉压和肺血管阻力升高的独立预测因子。受试者工作特征曲线分析结果显示miRNA-548a-5p的临界值为10.5,其表达水平升高,血流动力学恶化。结论血浆中miRNA-548a-5p可能是预测IPAH患者血流动力学恶化的生物标志物。Objective To study the value of miRNA-548a-5p in plasma of patients with idiopathic pulmonary arterial hypertension(IPAH)in predicting the increase of hemodynamic parameters.Methods The retrospective study included twelve IPAH patients and twelve control subjects who were hospitalized in Shanghai Pulmonary Hospital from May 2010 to June 2017.All of the patients underwent right-heart catheterization.Differential miRNA was screened in patients and controls by miRNA microarray.The effect of miRNA-548a-5p on increased hemodynamic parameters was analyzed by linear regression.Receiver operating characteristic curve(ROC)analysis was used to test the performance of miRNA-548a-5p in IPAH and control groups.Results There was no significant difference in baseline clinical parameters between IPAH patients and controls.The expression of miRNA-548a-5p was significantly higher in IPAH patients compared with controls(t=3.038,P<0.05).Some hemodynamic parameters were well correlated with the expression of miRNA-548a-5p.Multiple linear regression analysis suggested that miRNA-548a-5p level was an independent predictor of the increase of mean pulmonary artery pressure and pulmonary vascular resistance in patients with IPAH.ROC analysis showed that the cutoff value was 10.5.The increasing expression of miRNA-548a-5p predicted the deterioration of hemodynamic parameters.Conclusions Plasma miRNA-548a-5p may be a biomarker for predicting hemodynamic deterioration in patients with IPAH.
