白虎汤加减治疗儿童上呼吸道感染的疗效观察及对血清CRP、SAA、PCT的影响

摘　　要：目的探讨白虎汤加减对于儿童上呼吸道感染的临床疗效及其对血清C反应蛋白(CRP)、淀粉样蛋白A(SAA)、降钙素原(PCT)的影响。方法将80例上呼吸道感染患儿根据随机数字表法分为对照组和观察组各40例。对照组采用抗生素、抗病毒以及非甾体抗炎药物进行治疗,观察组采用白虎汤加减治疗。两组疗程均为3 d,疗程结束观察两组患儿临床疗效,统计治疗前后体温变化情况,以及血清CRP、SAA、PCT变化情况。结果两组患者治疗后对照组总有效率为92.5%,观察组为90.0%,两组比较差异无统计学意义(P>0.05)。两组患者在接受药物治疗后,体温及CRP、SAA、PCT水平与给药前比较均显著下降(P<0.05),而观察组与对照组治疗后同时段的体温及CRP、SAA、PCT水平比较差异无统计学意义(P>0.05)。结论白虎汤加减对于儿童上呼吸道感染的治疗有较高临床价值,其可以从多环节、多靶点治疗,包括抗病毒、抗细菌、抗炎等环节,以及CRP、SAA、PCT等靶点。白虎汤加减不仅仅在疗效上与西药无差异,而且在降低不良反应,减少耐药性方面,均有其独特优势。Objective To investigate the clinical efficacy of modified Baihu Decoction on children with upper respiratory tract infection and its effect on serum C-Reactive Protein(CRP),Serum Amyloid A(SAA),Procalcitonin(PCT).Methods Eighty children aged 1~12 years with upper respiratory tract infection were enrolled in the study.They were divided into control group and observation group according to the random number table method(40 cases per group).The patients in the observation group were treated with modified Baihu Decoction,and the control group was treated with antibiotics,antiviral drugs and non-steroidal anti-inflammatory drugs.The course of treatment lasted 3 days,and the changes in body temperature before and after the treatment were counted,as well as changes in serum CRP,SAA,and PCT.Results After treatment,the total effective rate of the control group was 92.5%and that of the observation group was 90.0%.There was no statistical difference between the efficacy of Modified Baihu Decoction and the clinical efficacy of western medicines(P>0.05).Modified Baihu Decoction and western medicines could effectively reduce the body temperature of children with upper respiratory tract infection(P<0.05).Both Modified Baihu Decoction and western medicines could effectively reduce the expression of CRP and SAA of children with upper respiratory tract infection(P<0.05).There was no statistically significant difference in PCT levels(P>0.05).Conclusion Modified Baihu Decoction has high clinical value in the treatment of children with upper respiratory tract infection.Modified Baihu Decoction can be used for multiple links,multi-target treatment of children with upper respiratory tract infection,including anti-viral,anti-bacterial,anti-inflammatory,as well as CRP,SAA,PCT and other targets.Modified Baihu Decoction is not statistically different from western medicines in its curative effect,and it has unique advantages in reducing side effects and drug resistance.
