腹腔镜保守性手术联合GnRH-a治疗卵巢巧克力囊肿的疗效研究

摘　　要：目的:探讨卵巢巧克力囊肿患者腹腔镜手术后应用GnRH-a的临床疗效及复发率。方法:回顾性分析2015-05~2017-10在我院行腹腔镜检查确诊并手术的卵巢巧克力囊肿40例,按照治疗方式不同分为对照组与观察组,每组20例。对照组患者给予腹腔镜保守性手术治疗,观察组患者在此技术上给予GnRH-a治疗,检查患者性激素水平,包括血清雌二醇(E2)、黄体生成素(LH)及卵泡刺激素(FSH)水平,对两组治疗方案的临床治疗效果作比较。结果:治疗前2组性激素水平比较,差异无统计学意义(P>0.05)。术后6个月随访发现,观察组LH、FSH、E2水平均明显低于对照组,差异有统计学意义(P<0.05,P<0.01)。观察组巧克力囊肿的复发率明显低于对照组,差异有统计学意义(5.0%vs 20.0%,P<0.05)。结论:给予卵巢巧克力囊肿患者腹腔镜保守性手术联合GnRH-a治疗方案,治疗效果显著,既可降低术后复发率又能调节激素水平。Objective:To investigate the clinical efficacy and recurrence rate of gnrh-a in patients with ovarian chocolate cyst after laparoscopic surgery.Methods:Forty cases of patients with ovarian chocolate cyst diagnosed by laparoscopy and operated in jiamusi central hospital from May 2015 to October 2017 were retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,with 20 cases in each group.Patients in the control group were treated with laparoscopic conservative surgery,and patients in the observation group were treated with gnrh-a on the basis of surgery in the control group,and their sex hormone levels,including serum estradiol(E 2),luteinizing hormone(LH)and follicular stimulating hormone(FSH),were checked,and the clinical efficacy and recurrence rate of the two groups were compared.Results:there was no significant difference in sex hormone level between the two groups before treatment(P>0.05).After 6 months of follow-up,it was found that the levels of LH,FSH and E 2 in the observation group were significantly lower than those in the control group,with statistically significant differences(P<0.05).The recurrence rate of chocolate cyst in the observation group was 5.0%,which was significantly lower than 20.0%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Laparoscopic conservative surgery combined with gnrh-a in the treatment of patients with ovarian chocolate cyst has a significant effect,which can not only reduce the rate of postoperative recurrence but also regulate hormone levels,which has certain clinical significance.
