锥颅穿刺引流术与钻孔穿刺引流术致穿刺径路出血率的比较研究

摘　　要：目的对比锥颅穿刺引流术与钻孔穿刺引流术致穿刺径路出血率。方法选择2016年2月—2019年2月我院收治的80例梗阻性脑积水患者作为研究对象,采用随机数字表法分为2组,各40例。A组采用锥颅穿刺引流术,B组采用钻孔穿刺引流术,比较2组手术指标、穿刺径路出血率、昏迷、预后情况。结果A组手术时间、术中出血量均低于B组,差异有统计学意义(P<0.05);A组穿刺径路出血发生率略低于B组,但差异无统计学意义(P>0.05);术后2组GCS评分均高于术前,且A组高于B组,差异有统计学意义(P<0.05);2组GOS分级对比,差异无统计学意义(P>0.05)。结论采用锥颅穿刺引流术与钻孔穿刺引流术均可改善梗阻性脑积水患者的昏迷程度,但锥颅穿刺引流术创伤更小,不增加穿刺路径出血发生率,可大大缩短手术时间,利于患者预后改善。Objective To investigate the bleeding rate of the puncture path caused by intracranial puncture drainage and trepanation puncture drainage.Methods 80 patients with obstructive hydrocephalus who admitted in the hospital from February 2016 to February 2019 were divided into two groups according to random number table,with 40 cases in each group.A group was given intracranial puncture drainage,and B group was given trepanation puncture drainage.The surgical indexes,bleeding rate of puncture path,coma and prognosis were compared between two groups.Results The operation time and intraoperative bleeding volume in A group were lower than those in B group(P<0.05);The incidence of bleeding of puncture path in A group was slightly lower than that in B group,but the difference was not statistically significant(P>0.05);After surgery,the GCS scores of two groups were higher than those before surgery,and A group was higher than B group(P<0.05);There was no statistical difference in the GOS grading between two groups(P>0.05).Conclusion Intracranial puncture drainage and trepanation puncture drainage can improve the degree of coma in patients.However,ntracranial puncture drainage has less trauma and does not increase the incidence of bleeding of puncture path,which can greatly shorten the operation time and facilitate the prognosis of patients.
