本院相关人群贮存式自体输血认知及意愿的调查分析

摘　　要：目的调查与分析泉州市某三甲医院相关人群对贮存式自体输血(PABD)的认知情况。方法选取泉州市某三甲医院的医护人员、患者及患者家属600例,采用自行设计问卷进行调查,问卷内容包括调查对象的一般信息、对自体输血的了解情况、对开展PABD的关注点,参加意愿及动机,并对调查结果进行统计分析。结果医务工作者对PABD认知及接受情况均优于非医务工作者。logistic回归分析显示:职业类别,文化程度,家族输血史是认知及接受情况的独立影响因素(95%CI分别为:0.021~0.082,1.242~4.474,0.041~0.146和0.233~0.780,1.358~3.998,0.121~0.379)。而关注点这个问题上,PABD对身体的影响和疗效是被调查者关注的焦点,除此之外医务人员关注点更侧重于血液浪费及输血反应等问题;而患者及其家属输血反应及相关费用问题的关注度较大。结论科室应通过更多方式加强患者及其家属乃至医护人员对自体输血的了解,医院也应采取适当措施以支持PABD的开展及宣传,对社会而言新闻自媒体等应尽可能增加自体输血知识的普及,加强其临床效益和社会效益的宣传,让更多人对自体输血有充分的知情权和选择权。Objective To investigate and analyze on the Cognition of preoperative autologous blood donation(PABD)among the related population in a 3A Hospital in Quanzhou City.Methods Six hundred medical staff,patients and family members of a 3A hospital in Quanzhou City were selected for investigation by self-designed questionnaire,including general information of the subject,understanding of PABD,attention of PABD,willingness and motivation of PABD,and statistical analysis of the investigation results.Results The cognition and acceptance of PABD among medical workers were better than those of non-medical workers.Logistic regression analysis showed that:Occupational category,educational level and family transfusion history are independent influencing factors of cognition and acceptance(OR,95%CI:0.021-0.082,1.242-4.474,0.041-0.146 and 0.233-0.780,1.358-3.998,0.121-0.379).On the issue of concern,the influence and curative effect of PABD on the body is the focus of the respondents′attention.In addition,medical staff pay more attention to blood waste and blood transfusion reaction;while patients and their families pay more attention to blood transfusion reaction and related costs.Conclusion Departments should strengthen the understanding of PABD by patients and their families and even medical staff,hospitals should also take appropriate measures to support the development and publicity of PABD,news self-media should increase the popularization of PABD knowledge as much as possible,strengthen the publicity of its clinical benefits and social benefits,so that more people have full right to know and choose PABD.
