连云港市地区健康人群血栓弹力图参考范围的建立

摘　　要：目的探索建立适合连云港市地区人群血栓弹力图(TEG)各参数的参考值范围。方法采集114例连云港市地区2018年4-8月健康成年志愿者静脉血4 mL,分别测定TEG和凝血四项,排除凝血四项测定的凝血功能异常者。应用北京乐普生产的血栓弹力图仪(CFMS LEPU-8800)分别测定以下参数:凝血反应时间(R)、凝血形成时间(K)、凝固角(α角)、最大振幅(MA)和凝血综合指数(CI),然后利用SPSS17.0统计学分析软件处理相关数据。结果114例志愿者TEG各参考指数正常参考值分别为R:4.86~10.45 min,K:1.17~4.53 min,α角:40.65°~73.45°,MA:48.05~72.82 mm,CI:-5.96~3.17。检测指标中43.0%(49/114)志愿者至少有一项超出厂家提供的参考范围。约有20.2%志愿者被诊断为凝血异常,其检测特异性71.4%。结论连云港市地区健康人群的血栓弹力图检测结果与厂家提供的正常参考值范围存在差异,本研究建立符合本地区人群特点的参考值范围,可以为广大实验室提供参考借鉴,为临床科室涉及出凝血功能领域的基础理论研究或疾病的诊治与监测提供帮助。Objective To explore the establishment of a reference range for the parameters of thromboelastogram(TEG)for people in Lianyungang City.Methods A total 114 of venous blood samples from healthy adult volunteers were collected from April to August 2018 in Lianyungang Cityand four items of TEG and coagulation were measured respectively to exclude those who with abnormal coagulation function.The thrombelastograph Coagulation Analyzer(CFMS lepu-8800)made by Beijing Lepu was used to measure the following parameters:coagulation reaction time(R),coagulation formation time(K),coagulation angle(α-Angle),maximum amplitude(MA)and coagulation index(CI),and processed relevant data with SPSS 17.0 statistical analysis software.Results The normal reference values of the reference indices of 114 TEG volunteers were R:4.86-10.45 min,K:1.17-4.53 min,α-Angle:40.65°-73.45°,MA:48.05-72.82 mm,CI:-5.96-3.17.At least one of the 43.0%(49/114)volunteers in the test indicators exceeded the reference range provided by the manufacturer.Approximately 20.2%of the volunteers were diagnosed as coagulopathy and the detection specificity was 71.4%.Conclusion The results of thromboelastography in healthy people in Lianyungang city are different from the range of normal reference values provided by the manufacturer.This study establishes a reference value range in line with the local population,which can provide reference for the majority of laboratories,and provide help for the clinical departments involved in the basic theoretical research in the field of coagulation function or the diagnosis,treatment and monitoring of diseases.
