摘 要:目的探讨乙肝病毒血清标志物和乙肝病毒脱氧核糖核酸(HBV-DNA)以及肝功能指标的相关性,提高乙肝患者诊断的准确性。方法选取2016年1月至2017年8月于该院行乙肝病毒检测的400例患者,应用时间分辨荧光免疫分析法(TRFIA)检测乙肝病毒血清标志物,包括乙肝表面抗原(HBsAg)、乙肝表面抗体(HBsAb)、乙肝e抗原(HBeAg)、乙肝e抗体(HBeAb)和乙肝核心抗体(HBcAb);应用实时荧光定量聚合酶链反应(FQ-PCR)检测HBV-DNA;采用连续监测速率法检测肝功能指标,包括丙氨酸氨基转移酶(ALT)和天门冬氨酸氨基转移酶(AST);分析评价以上3者的关联性。结果400例患者HBV血清标志物的检测结果有13种模式,其中“小三阳”,即145阳性模式,检出率最高,为54.75%;HBV-DNA水平在HBsAg阳性组与阴性组间、HBeAg阳性组与阴性组间均有明显差异(均P<0.01);AST异常率在HBeAg阳性与阴性组间差异有统计学意义(P<0.05);高水平的HBV-DNA主要存在于“大三阳”,即135阳性模式(P<0.01);ALT和AST的异常率随着HBV-DNA水平的升高而不断增加(均P<0.01)。结论乙肝病毒血清标志物、HBV-DNA以及肝功能指标的联合检测,有助于临床及时准确地诊断乙肝病毒感染。Objective To discuss the correlation of hepatitis B virus(HBV)serologic markers,HBV-DNA and liver function indexes.Methods Four hundred patients done the HBV test from January 2016 to August 2017 were enrolled.Serologic markers including surface antigen(HBsAg),surface antibody(HBsAb),e antigen(HBeAg),e antibody(HBeAb)and core antibody(HBcAb)were Analyzed by time-resolved fluoroimmunoassay(TRFIA).HBV-DNA was Analyzed by real-time fluorescence quantitative polymerase chain reaction(FQ-PCR).Liver function indexes including alanine aminotransferase(ALT)and aspartate transaminase(AST)were Analyzed by continuous monitoring rate method.The relevance of the above three method results were Analyzed and evaluated.Results There are 13 patterns of HBV serum markers in 400 patients,among which the positive rate of“Xiao San Yang”145 pattern was the highest,it was 54.75%;There were significant differences in HBV-DNA between HBsAg positive group and negative group,HBeAg positive group and negative group(P<0.01));the differencet of AST abnormal rate between HBeAg negative and HBeAg positive was statistically significant(P<0.05);high level of HBV-DNA was mainly found in 135 positive pattern(P<0.01);the abnormal rates of ALT and AST increased with the increase of HBV-DNA level(P<0.01).Conclusion The combined detection of HBV serum markers,HBV-DNA and liver function indexes is helpful for the timely and accurate diagnosis of HBV infection.
