摘  要:目的:探讨非哺乳期乳腺炎患者急性炎症期外周血中性粒细胞(N)、自然杀伤细胞(NK)及树突状细胞(DC1、DC2)水平的变化情况。方法:收集2017年7月—2019年7月深圳市龙岗区人民医院收治的25例非哺乳期乳腺炎急性炎症期患者(非哺乳期乳腺炎组)和25例健康体检者(对照组),检测其外周血N、NK、DC1、DC2的水平,采用t检验比较非哺乳期乳腺炎组与对照组间各指标的差异,用单因素方差分析比较非哺乳期乳腺炎组有无先天性乳头内陷、不同哺乳情况、不同发病年龄及不同乳房炎症包块面积各亚组与对照组间外周血N、NK、DC1及DC2的差异。结果:非哺乳期乳腺炎组外周血中的N、NK及DC2水平均高于对照组(P均<0.010),DC1水平低于对照组(P<0.05)。有无先天性乳头内陷组、有无哺乳组、不同发病年龄组及不同乳房炎症包块面积组各亚组N、NK细胞水平均高于对照组(P<0.001),DC1细胞的水平均低于对照组(P<0.001)。有先天性乳头内陷组DC2细胞水平高于对照组(P<0.001)及无先天性乳头内陷者组(P<0.05)。有哺乳组DC2细胞水平高于对照组(P<0.001)。非生育高峰年龄(36~47岁)组中DC2细胞水平高于对照组(P<0.001)及生育高峰年龄(24~35岁)组(P<0.05)。不同乳房炎症包块组DC2细胞水平均高于对照组(P<0.05),炎症包块面积≥12.00 cm2组DC2细胞水平高于炎症包块<12.00 cm2组(P<0.05)。结论:非哺乳期乳腺炎患者急性炎症期的发生可能与体内的固有免疫功能紊乱有关,其紊乱程度可能与乳头内陷、有哺乳史、发病年龄及乳房炎症程度有关。Objective:To investigate the changes of Neutrophil cells(N),Natural killer cells(NK)and Dendritic cells(DC1,DC2)in peripheral blood of the non-puerperal mastitis patients in acute infla mmation peroid. Methods: From July 2017 to July 2019, 25 patients with acute non-puerperal mastitis (non-puerperal mastitis group) and 25 healthy people(control group) were enrolled in Shenzhen Longgang District People's hospital.The expression levels of N, NK, DC1, DC2 were determined respectively. T test was used to compare the differences between the non-puerperal mastitis group and the normal control group. One-way factor analysis of variance was performed to analyze the difference between the subgroup of non- puerperal mastitis patients who were grouping according to congenital subsidence of nipples, fertility history,the age of onset and inflammatory breast agglomerate area and the control group with the levels of N, NK, DC1 and DC2 in peripheral blood. Results: The levels of N, NK and DC2 cells in peripheral blood in the non-puerperal mastitis patients in the acute inflammation peroid were higher than those in the control group(P<0.001), and the level of DC1 cells was lower than those in the control group(P<0.005). The levels of N and NK cells levels were significantly higher in the groups with or without congenital subsidence of nipples , with or without fertility history, with or without at fertile peak age, in different inflammatory breast agglomerate area than those in the control group(P<0. 001), and the levels of DC1 were significantly lower(P<0.05)than those in the control group.The levels of DC2 in the group with congenital subsidence of nipples were significantly higher than those in the control group(P<0.001) or the group without congenital subsidence of nipples(P<0.05). The levels of DC2 in the fertility history group were significantly higher than those in the control group(P<0.001).
