308nm准分子光联合0.1%他克莫司软膏治疗稳定期白癜风的疗效及安全性分析

摘　　要：目的:分析308nm准分子光联合0.1%他克莫司软膏治疗稳定期白癜风的疗效及安全性。方法:回顾性分析笔者医院收治的121例稳定期白癜风患者,按治疗方式不同分成两组。单一组:50例,予以308nm准分子光治疗;联合组:71例,予以308nm准分子光联合0.1%他克莫司软膏治疗,治疗3个月,比较两组临床疗效、皮肤CT情况、Foxp3 mRNA表达水平以及细胞因子水平,并观察其安全性。结果:治疗3个月,联合组总治愈率为76.06%,显著高于单一组的56.00%(P<0.05);联合组部分黑素细胞活跃53例(74.65%),单一组部分黑素细胞活跃16例(32.00%),联合组发生率显著高于单一组(P<0.05);两组Foxp3 mRNA表达水平显著上调(P<0.05),联合组上调幅度大于单一组(P<0.05);两组TNF-α、INF-γ显著降低(P<0.05),SCF、bFGF显著升高(P<0.05),联合组变化幅度大于单一组(P<0.05);两组血常规、肝肾功能无明显异常,治疗期间不良反应发生率无显著性差异(P>0.05)。结论:308nm准分子光联合0.1%他克莫司软膏治疗稳定期白癜风临床疗效明确,可显著改善患者皮损症状,上调Foxp3 mRNA表达,调节细胞因子水平,安全有效。Objective To analyze the efficacy and safety of 308 excimer light combined with 0.1%tacrolimus ointment in the treatment of stable vitiligo.Methods A retrospective analysis was performed on 121 patients with stable vitiligo in our hospital,and they were divided into two groups according to different treatment methods.50 patients in single group were given 308 excimer light therapy while 71 patients in combined group were given 308 excimer light combined with 0.1%tacrolimus ointment,and they were treated for 3 months.The clinical efficacy,skin CT,expression level of Foxp3 mRNA and levels of cytokines were compared between the two groups,and the safety was observed.Results After 3 months of treatment,the total cure rate in combined group was significantly higher than that in single group(76.06%vs 56.00%)(P<0.05).There were 53 cases(74.65%)of some active melanocytes in combined group and 16 cases(32.00%)in single group,and the incidence rate of some active melanocytes in combined group was significantly higher than that in single group(P<0.05).The expression level of Foxp3 mRNA was significantly up-regulated in the two groups(P<0.05),and the up-regulation in combined group was greater than that in single group(P<0.05).The TNF-αand INF-γwere significantly decreased in the two groups(P<0.05)while the SCF and bFGF were significantly increased(P<0.05),and the changes in combined group were larger than those in single group(P<0.05).There were no obvious abnormalities in the blood routine and liver and kidney function in the two groups,and there was no significant difference in the incidence rate of adverse reactions during treatment(P>0.05).Conclusion 308 excimer light combined with 0.1%tacrolimus ointment has exact clinical efficacy in the treatment of stable vitiligo,and it can significantly improve the symptoms of skin lesions,up-regulate Foxp3 mRNA expression,and regulate cytokines levels,and it is safe and effective.
