摘 要:目的:探讨富血小板纤维蛋白(Platelet-rich fibrin,PRF)修复难愈性骨外露创面的临床疗效。方法:将2016年4月-2019年3月在达州市中心医院烧伤整形科住院和伤口中心门诊治疗的骨外露60例患者随机分为实验组(PRF治疗)和对照组(藻酸盐敷料+表皮生长因子治疗),比较两组治疗后2周的创面肉芽组织覆盖情况、肉芽组织覆盖全部创面时间、换药次数。结果:实验组治疗2周后骨外露面积缩小(4.6±1.12)cm2明显高于对照组(3.6±0.61)cm2,肉芽组织全覆盖时间为(20.5±6.15)d少于对照组(37.4±9.13)d,换药次数(3.83±1.09)次低于对照组的(15.86±4.28)次,差异均有统计学意义(P<0.05)。结论:PRF能明显促进肉芽组织生长,缩短骨外露创面肉芽覆盖时间,减少换药次数,可以作为骨外露创面的治疗方法之一,值得临床推广应用。Objective To investigate the clinical therapeutic effect of platelet-rich fibrin(PRF)in repairing refractory bone exposure wounds.Methods From April 2016 to March 2019,Sixty patients with bone exposure treated in the department of burn and plastic surgery of Dazhou Central Hospital were randomly divided into the experimental group(PRF treatment)and the control group(alginate dressing+epidermal growth factor treatment).The granulation tissue coverage,granulation tissue coverage time and dressing change times were compared between the two groups 2 weeks after treatment.Results After 2 weeks of treatment,the bone exposure area of the experimental group decreased(4.6±1.12)cm2,which was significantly higher than that of the control group(3.6±0.61)cm2,the granulation tissue covered time was(20.5±6.15)d,which was shorter than(37.4±9.13)d in the control group,the frequency of dressing change(3.83±1.09)was lower than that of control group(15.86±4.28),the differences were statistically significant(P<0.05).Conclusion PRF can significantly promote the growth of granulation tissue,shorten the time of granulation coverage on open refractory bone wounds,and reduce the frequency of dressing changes.It can be used as one of the treatment methods for exposedrefractory bone wounds and is worthy of the clinical application and promotion.
