分流管庆大霉素浸泡联合预防性静脉应用头孢曲松钠降低脑室腹腔分流术后感染6例分析

摘　　要：目的总结分流管庆大霉素浸泡联合预防性静脉应用头孢曲松钠降低脑室腹腔分流术后感染率。方法对6例脑室腹腔分流术患者于术前1天及手术当天静脉应用头孢曲松钠2 g,术中分流管盐酸庆大霉素8万单位3支配生理盐水浸泡30 min。结果术后6例患者全部无感染,术口干洁,影像复查脑室明显缩小,痴呆好转,步态不稳和尿失禁基本消失。结论分流管庆大霉素浸泡联合预防性静脉应用头孢曲松钠能降低脑室腹腔分流术后感染率,效果确切,累积案例后形成成果后值得推广。Objective To analyze the effect of shunt tube gentamicin immersion combined with prophylactic intravenous application of ceftriaxone sodium in reducing the infection rate after ventriculoperitoneal shunt.Methods Six patients with ventriculoperitoneal shunt were given 2 g of ceftriaxone sodium intravenously 1 day before surgery and on the day of surgery.The shunt tube was immersed in normal saline for 30 minutes with 80000 units of gentamycin hydrochloride.Results All these patients were free of infection after the operation.The operative wound was dry and clean.The images showed that the brain ventricle was significantly reduced.The symptoms of dementia were relieved,and unstable gait and urinary incontinence basically disappeared.Conclusion Shunt tube gentamicin immersion combined with prophylactic intravenous application of ceftriaxone sodium can reduce the infection rate after ventriculoperitoneal shunt,with definite effect,which is worth promoting after the accumulation of cases.
