多部门联合急救演练在产科急重症救治中的应用

摘　　要：目的分析多部门联合急救演练在产科急重症救治中的应用效果。方法回顾性分析2015年1月至2019年12月产科分娩的27151例产妇,其中产房急手术2423例临床资料,于2017年开始于产科实施多部门联合急救演练,对比实施前后急重症救治效果。结果2015年新生儿窒息率为2.1%、2016年新生儿窒息率为2.2%、2017年新生儿窒息率为1.5%、2018年新生儿窒息率为1.7%、2019年新生儿窒息率为1.8%,五年间新生儿窒息率相比较,差异无统计学意义(P>0.05),但自2017年起呈现出明显的下降趋势;2015年产房急手术产妇由决定手术到开始时间为(32.50±1.50)min、2016年为(30.00±1.00)min、2017年为(12.25±1.25)min、2018年为(12.00±1.00)min、2019年为(10.00±1.50)min,五年相比较,2015年与2016年长于2017年、2018年、2019年,差异均有统计学意义(均P<0.05),2015年与2016年、2017年和2018年与2019年相比较,差异均无统计学意义(均P>0.05)。结论在产科急重症救治中多部门联合急救演练有助于降低新生儿窒息率、缩短由决定手术到手术开始时间,值得推广使用。Objective To analyze the effect of multi-sector joint emergency drill in emergency treatment of obstetrics.Methods A retrospective analysis of clinical data of 27,151 women who delivered in the obstetrics department of our hospital from January 2015 to December 2019 was carried out,including 2,423 cases of emergency delivery in the delivery room.The multi-departmental joint emergency drill was performed in the obstetric department since 2017.The effects of emergency treatment before and after the implementation were compared.Results The neonatal asphyxia rate was 2.1%in 2015,2.2%in 2016,1.5%in 2017,1.7%in 2018,and 1.8%in 2019,there was no statistically significant difference in the neonatal asphyxia rate among the five years(P>0.05),but it showed a significant downward trend since 2017.The time from making decision to surgery of emergency puerperae was(32.50±1.50)min in 2015,(30.00±1.00)min in 2016,(12.25±1.25)min in 2017,(12.00±1.00)min in 2018,(10.00±1.50)min in 2019,those in 2015,2016 were longer than those in 2017,2018,and 2019,with statistically significant differences(all P<0.05),but there were no statistically significant differences between 2015 and 2016,and among 2017,2018,and 2019(all P>0.05).Conclusion Multi-sector joint emergency drill in acute and severe treatment of obstetrics can help reduce the rate of neonatal asphyxia and shorten the time from the decision of surgery to the start of surgery,which is worth popularizing.
