咳喘散热疗对间质性肺疾病病人呼吸功能及生活质量的影响

摘　　要：[目的]探讨咳喘散封包热疗对间质性肺疾病病人呼吸功能及生活质量的影响。[方法]采用随机数字表法将符合纳入标准的病人随机分为对照组和试验组,本研究共纳入84例病人,其中对照组40例,试验组44例,对照组采用常规治疗,试验组在常规治疗的基础上增加咳喘散封包热疗,每天2次,每次持续60 min,并在入院48 h内、出院12周后分别搜集病人的6 min步行试验、肺痿中医证候积分表、中文版King′s间质性肺疾病简短问卷(K-BILD)、改良英国MRC呼吸困难指数(mMRC)、3个月内急诊就诊次数和住院次数,以评测其干预效果。[结果]干预前两组病人的一般资料比较差异均无统计学意义(P>0.05),干预后试验组6 min步行试验、肺痿中医证候积分表、K-BILD、mMRC、3个月内急诊就诊次数和住院次数比较差异均有统计学意义(P<0.05)。[结论]对于临床收治的间质性肺疾病病人,使用咳喘散于背俞穴热疗可提高病人的6 min步行距离,降低肺瘘中医证候积分,改善呼吸困难程度以及提高间质性肺疾病病人的生活质量,降低病人3个月内的就诊次数及住院次数。Objective:To explore the effect of hyperthermia therapy with Kechuan San packet on respiratory function and quality of life(QOL)in patients with interstitial lung disease(ILD).Methods:A total of 88 patients who met the inclusion criteria were included in the study.They were randomly divided into the control group(40 cases)and the experimental group(44 cases)by using the randomized digital table method.The patients in control group received routine treatment while the patients in experimental group received hyperthermia therapy with Kechuan San packet twice a day for 60 minutes each time on the basis of routine treatment.Within 48 hours of admission and at 12 weeks after discharge from the hospital,the results of patients′6-minute walking test(6 MWT),Chinese medicine symptom complex score for consumptive lung disease,Chinese version of The King′s Brief Interstitial Lung Disease Questionnaire(KBILD),Modified British Medical Research Council(mMRC),and emergency visits and hospitalization times within 3 months were collected and used to evaluate the effect.Results:Before the intervention,there was no significant difference between the two groups(P>0.05).After the intervention,the differences were statistically significant in 6 MWT,Chinese medicine symptom complex score for consumptive lung disease,the Chinese version of KBILD,mMRC,emergency visits,and hospitalization times between the two groups(P<0.05).Conclusions:For clinically patients treated with ILD,the Hyperthermia Therapy with Kechuan San Packet at Beishu acupoint could increase the distance of 6 MWT,the score of Chinese medicine symptom complex score for consumptive lung disease,improve the degree of dyspnea and the QOL of patients with ILD,reduce the number of patients′visits and hospitalizations within 3 months.
