肺复张联合有创-无创序贯性机械通气治疗急性呼吸窘迫综合征的疗效分析

摘　　要：目的分析肺复张联合有创-无创序贯性机械通气治疗ARDS(急性呼吸窘迫综合征)的临床疗效。方法纳入病例是濮阳市安阳地区医院2016年7月-2019年7月收治的92例ARDS患者,根据治疗方法不同分为两组,参照组46例患者采纳有创-无创序贯性机械通气治疗,实验组46例患者在参照组基础上联合肺复张治疗,对比两组临床疗效、呼吸力学指标、VAP发生率。结果实验组临床总有效率(95.65%)显著比参照组(73.91%)高;实验组治疗48h后Pplat、PIP水平均显著比参照组低,Cstat水平显著比参照组高;实验组VAP发生率(2.17%)显著比参照组(15.22%)低(P<0.05)。结论肺复张联合有创-无创序贯性机械通气治疗ARDS疗效显著,可改善患者呼吸力学指标,降低VAP发生率。Objective To analyze the clinical effect of lung recruitment combined with invasive-noninvasive sequential mechanical ventilation in the treatment of ARDS(acute respiratory distress syndrome).Methods 92 patients with ARDS admitted to Anyang district hospital in Puyang from July 2016 to July 2019 were divided into two groups according to different treatment methods.46 patients in the control group were treated with invasive-noninvasive sequential mechanical ventilation,the 46 patients in the experimental group were treated with pulmonary resuscitation on the basis of the control group.The clinical effect,respiratory mechanics index and the incidence of VAP were compared between the two groups.Results The total effective rate(95.65%)of the experimental group was significantly higher than that of the control group(73.91%),the level of PPLAT and PIP in the experimental group was significantly lower than that of the control group and the level of CSTAT in the experimental group was significantly higher than that of the control group after 48 hours of treatment.The incidence of VAP in the experimental group(2.17%)was significantly lower than that in the control group(15.22%)(P<0.05).Conclusion Lung recruitment combined with invasive-noninvasive sequential mechanical ventilation is effective in treating ARDS,which can improve respiratory mechanics index and reduce the incidence of VAP.
