超早期与早期手术对基底核区高血压脑出血患者的效果影响分析

摘　　要：目的探讨超早期与早期手术对基底核区高血压脑出血患者的效果。方法回顾分析开封市人民医院2014年1月-2019年9月收治的80例基底核区高血压脑出血患者的临床资料,根据发病治疗时间不同分为两组,包括早期组(7~12h)及超早期组(<7h)组,各40例。80例患者均给予小骨窗显微手术,其中超早期组患者在发病后<7h进行手术,早期组患者在发病7~12h进行手术,对比两组患者治疗后的GOS评分、ADL评分及血肿消散时间,对比两组患者的并发症发生率。结果超早期组的GOS评分明显高于早期组,超早期组的工具性日常生活能力评分、躯体生活自理量表评分及血肿消散时间明显低于早期组(P<0.05)。超早期组的并发症发生率为12.5%,早期组为35.0%,超早期组的并发症发生率明显低于早期组(P<0.05)。结论与早期手术相比,对基底核区高血压脑出血患者行超早期手术,可改善患者的预后,提高患者的生活能力,降低并发症发生率。Objective To investigate the effect of ultra-early and early operation on hypertensive cerebral hemorrhage in basal ganglia region.Methods The clinical data of 80 patients with hypertensive cerebral hemorrhage in basal ganglia from January 2014 to September 2019 in Kaifeng people’s hospital were analyzed retrospectively,the patients were divided into two groups according to the time of treatment,including the early group(7~12 h)and the ultra-early group(<7 h),40 cases each.All the 80 patients were treated with small bone window microsurgery,the patients in the super early group were operated on less than 7 hours after onset,and the patients in the early group were operated on from 7 to 12 hours after onset.The GOS score,ADL score and hematoma resolution time after treatment were compared between the two groups,the incidence of complications was compared between the two groups.Results The scores of GOS in the super-early group were significantly higher than those in the early group.The scores of the instrumental ADL,the physical self-care scale and the time of evacuation of hematoma in the super-early group were significantly lower than those in the early group(P<0.05).The incidence of complications was 12.5%in the early group and 35.0%in the early group.The incidence of complications in the early group was significantly lower than that in the early group(P<0.05).Conclusion Compared with the early operation,the super-early operation on hypertensive intracerebral hemorrhage in basal ganglia can improve the prognosis,improve the life ability and reduce the complication rate.
